Acute ischemic stroke requires timely evidence-based treatment to mitigate long-term disability and reduce costs. In 2003, the American Heart Association (AHA)/American Stroke Association (ASA) launched Get With the Guidelines (GWTG)–Stroke,1 a national voluntary stroke registry program, to improve stroke care(NOT RECOVERY!) quality. Twenty years on, we evaluated the outcomes of quality improvement in 9 AHA/ASA guideline-recommended stroke care quality measures among US patients who have had an acute ischemic stroke using a cost-effectiveness analytic framework. Our study includes retrospective and prospective analyses to assess the past performance and potential of these measures.
