Impact of Roy adaptation model-based nursing combined with rehabilitation training on functional status and quality of life in patients with hemiplegia after stroke: A randomized controlled trial

Objective: 

The Roy Adaptation Model (RAM) guides healthcare providers in assessing stimuli behind adaptation issues, enabling tailored interventions to reduce ineffective responses. Therefore, we investigated the effects of nursing based on RAM combined with rehabilitation training on functional status and quality of life in patients with hemiplegia after stroke. 

Methods: 

This is a randomized, controlled, and single-center study. Patients with hemiplegia after stroke admitted to Funan County People's Hospital from October 2023 to October 2024 were randomly classified into a control group ((64.70 ± 3.98) years, n = 50, received routine nursing and rehabilitation training) and an observation group ((65.00 ± 3.82) years, n = 50, received RAM-based nursing and rehabilitation training). Outcomes were assessed using the Fugl-Meyer Assessment (FMA), Exercise of Self-Care Agency Scale (ESCA), Activity of Daily Living Scale (ADL), Stroke-Specific Quality of Life Scale (SS-QOL), Self-Rating Anxiety Scale (SAS), and Self-Rating Depression Scale (SDS) before and one month after the intervention. Patient satisfaction with nursing care and the incidence of complications were recorded. 

Results: 

Compared with baseline, both groups showed improvements in FMA, ESCA, ADL, and SS-QOL scores, and decreases in SAS and SDS scores, with the observation group exhibiting greater improvements. There exists a group × time interaction effect on the nursing effectiveness of relevant scores. The observation group also had higher patient satisfaction, and lower incidence of complications (X2=4.332, P = 0.037). 

Conclusion: 

For post-stroke hemiplegia, RAM-based nursing integrated with rehabilitation training comprehensively enhances recovery outcomes, improves quality of life, and achieves high patient satisfaction.
