How has stroke changed your life? questionaire from NSA

Just got this as an email. Extremely disappointed in it. No space for written answers. No specificity to any of the questions. There is nothing in any of the answers received that could be used to create and fill survivor needs. Taryn, I expected better, something closer to the British Stroke Association.
Basic Stationery Top - Redesign 2010
Dear dean,
We like to start the year off right, and for us at National Stroke Association, that means equipping ourselves with all of your needs as a person interested in knowing more about stroke.
We readied our anonymous Annual Community Needs Survey project earlier than ever so we can make sure our programs and resources in 2013 are designed to fit your current needs.
I invite you to take our annual survey by clicking here now. The questions are designed to tell us more about how you learn, how you wish to receive information and how we can better serve your needs. It will only take a few minutes and it will help shape all of the outreach we do for you and others like you.
Many of our staff and board members have helped develop this important survey project and we have put much thought into how we can best ensure your needs are met. Your feedback is most valued and we are grateful for your support as we continue to identify ways to better serve the stroke community at large.
Thank you for your time.
Warm regards,
