Dear dean,
The first
days at home after a stroke are confusing and overwhelming. With your support,
we offer real help.
By making a gift today, you create meaningful impact in the
lives of stroke survivors and their vital circle of care. And, because of our
matching gift challenge, your gift will go twice as far to meet the critical
needs that stroke survivors face.
Through
programs such as our Stroke Recovery
Navigatorsm survivors
and caregivers partner with a trained Patient Navigator for up to six months
after discharge from the hospital. Given that each survivor’s recovery journey
is unique, it is vital that that the Navigator and survivor co-create a plan to
identify and overcome barriers, attain self-advocacy skills, and access support
and resources. Our groundbreaking program empowers survivors to take control of
their recovery. With the support of this program:
- 4% of stroke survivors were readmitted to the hospital due to stroke, compared to a recent study average of 15%
- 99% of stroke survivors reported an increased ability to resume the activities they previously engaged in socially and within their community
- 94% of caregivers reported a decrease in the burden created when they stepped into the caregiving role
Will you make a gift today to support
this important work?
Don’t forget – through the generosity of an anonymous donor, your donation will
be doubled to make twice the impact for our stroke community. In order for this
program and our other life-saving initiatives to move forward, we need your gifts. Thank you for your support.
Warm Regards,
Jamie
