World War II literally ran on speed—Germans had Pervitin (methamphetamine they nicknamed “tank chocolate”), while Allies distributed Benzedrine, basically 1940s Adderall.

The Air Force fed pilots “go pills”—pure dextroamphetamine—from Vietnam through Afghanistan, which may have contributed to a 2002 incident where two US pilots on Dexedrine accidentally bombed a Canadian unit.

The pattern never changes: every shortcut works, but every shortcut bills you—whether it’s your heart, sleep, or addiction risk.

Enter Soviet Neuroscience

Semax is a heptapeptide—seven amino acids strung together: methionine, glutamate, histidine, phenylalanine, proline, glycine, and proline.

Unlike other peptides extracted from animal tissue, Semax was designed on purpose by Soviet pharmacologists at Moscow’s Institute of Molecular Genetics starting in the 1980s.

Researchers started with a fragment of ACTH (adrenocorticotropic hormone), which normally tells adrenal glands to produce cortisol. A four-amino-acid fragment showed positive stress effects on brain function—think learning, attention, resistance to low oxygen—without hormonal downsides like muscle breakdown.

Scientists welded on a proline-glycine-proline tail to prevent enzymatic degradation, creating a brain-targeted stress hormone fragment with built-in stabilizer. A non-stimulant cognitive booster with no obvious addiction profile—exactly what everyone had been hunting for.

At roughly 814 daltons, Semax is relatively small (BPC-157 is about 1,419, semaglutide exceeds 4,000). Small enough that Russians believed nasal mucous membranes could deliver it directly to brain tissue—hence the nasal drop delivery method.

How Semax Actually Works

The headline mechanism involves BDNF—brain-derived neurotrophic factor. Think of it as fertilizer for neurons, helping them survive, connect, and rewire.

In rat studies, Semax increases BDNF and its receptor TrkB in hippocampus and forebrain. That’s the most coherent thread in Semax research and the basis for every neuroplasticity claim attached to it.

It also nudges monoamine neurotransmitters like dopamine and serotonin. But here’s critical nuance matching user reports: Semax is not a stimulant.

In rat studies, Semax alone barely moved dopamine but amplified response when something else was already driving it. That’s why most users describe feeling more motivation rather than a true buzz.

It’s Not NZT

The entire Limitless premise builds on the myth that we only use a fraction of our brains. Unfortunately, that’s fiction—we’re already using essentially 100% of our brains at baseline.

Semax doesn’t unlock dormant capacity waiting for activation. It optimizes signaling in tissue you’re already running.

What Does Science Actually Show

The evidence is present but lopsided in a very specific way.

Bench and animal data is surprisingly deep. Cell studies show BDNF and nerve growth factor genes activating. Rat stroke research is strongest—researchers literally tied off brain arteries in rats, administered Semax, then watched gene expression shift from inflammatory/injury patterns toward repair.

One study showed boosted cell proliferation in injured rat brains, spawning online claims that “Semax regrows your brain.” But again, this is in mice—not 35-year-old humans trying to focus on Excel spreadsheets.

Human Data Is Thin and Russian

A small fMRI study showed intranasal Semax measurably changed brain network activity in healthy volunteers. Older EEG and cognitive work from the 1990s exists, plus a stroke rehab study where Semax plus early rehabilitation raised blood BDNF and slightly accelerated recovery.

But large placebo-controlled trials examining cognitive improvements—how most people actually use it—are still lacking.

Russia has used Semax as an approved drug for over 30 years in stroke patients, giving it more real-world data than almost any non-FDA-approved substance. While not as robust as semaglutide trials, in some ways it’s better—GLP-1 agonists haven’t been around nearly as long.

Semax’s efficacy as cognitive booster may be fuzzy, but its safety profile appears solid.

Dosing: Russian Medicine vs. Gray Market

Russian pharmaceutical Semax comes as nasal drops in two strengths: 0.1% solution for cognitive boost and stroke recovery, and stronger 1% reserved for acute hospital stroke treatment.

One drop of 0.1% delivers approximately 50 micrograms. Labeled daily range runs from a few hundred up to couple thousand micrograms, usually for only a few days at a time.

Gray market biohackers report using nasal sprays between 100-1,000 micrograms daily, typically 5 days on, 2 days off, because tolerance develops quickly and spray isn’t cheap.

Some insist they get better results from subcutaneous injection, though this has no obvious upside and adds another needle to weekly routine.

The Gray Market Problem

When buying “research use only” Semax, there’s decent chance you’re not buying Semax itself. There’s an entire family of remixes: N-acetyl Semax and N-acetyl Semax amidate (Adamax), each with chemical tweaks to extend half-life or intensify effects.

Those aren’t the molecules from Russian studies. Your mileage may vary—like buying a live bootleg concert recording where maybe it sounds better than studio version, or maybe the artist is drunk and slurring.

Side Effects and Safety

Semax is generally well-tolerated, which is major part of its appeal. But well-tolerated doesn’t mean risk-free.

Common user-reported complaints include:

  • Anxiety and irritability
  • Headaches
  • Nausea
  • Insomnia if taken late in day
  • Unpleasant emotional intensity—like volume got cranked on whatever you were already feeling

The single most common complaint? Feeling absolutely nothing, even at high doses. Could mean bad product, incomplete absorption, wrong analog, or simply individual non-response.

Russian labeling contraindicates pregnancy, breastfeeding, acute psychiatric illness, and notably, history of seizures. Anything nudging brain excitability deserves respect—”naturally derived peptide” doesn’t automatically mean benign.

The scariest thing about Semax in the USA isn’t the peptide itself—it’s the bottle. Since it can’t be legally manufactured here for human consumption, there’s zero guarantee regarding purity, sterility, or whether labeled dose matches actual contents.

FDA Meeting Could Change Everything

On July 23-24, the FDA’s Pharmacy Compounding Advisory Committee (PCAC) meets to decide the fate of several peptides, including Semax.

BPC-157, KPV, TB-500, and MOTS-c are up day one. Semax joins Epitalon and Dihexa on day two, July 24th.

These peptides exist in regulatory limbo. This meeting decides whether they return to a list allowing compounding pharmacies to legally manufacture them under real oversight with quality control, or stay banished to gray market forever.

For Semax specifically, the committee reviews it for cerebral ischemia and trigeminal neuralgia. While PCAC votes are technically advisory, FDA won’t grant access to legitimate high-quality Semax from compounding pharmacies without it.

There’s a public comment docket at regulations.gov that committee members must consider. Comments submitted by July 9th get physically handed to committee before the meeting. After that deadline, FDA may still read them, but people casting votes won’t see them.

The Real Limitless Pill

Is Semax a real-life limitless pill? Probably not. We’re still waiting for actual NZT, and nothing flips your brain to 100% because you’re already running at 100%.

Remember how Limitless actually ends: even fictional NZT had a body count—tolerance, blackouts, withdrawal, people dying when supply runs dry. Even the fantasy came with a bill.

But is Semax a real molecule with genuine cognitive effects, legitimate neuroprotective properties, and 30 years of Russian clinical use? Absolutely. It emerged from legitimate pharmaceutical research in Moscow, not a Hollywood writer’s room.

Semax may be subtle and context-dependent, but positive signals appear exactly where they matter—in cognition and the injured, stressed brain.

Your Real NZT Already Exists

For those chasing focus, the closest thing to a real-life limitless pill is less exciting than movie trailers but costs nothing: sleep, blood pressure control, exercise, avoiding alcohol, treating sleep apnea, addressing depression.

That’s your NZT. Once those fundamentals are dialed, Semax might be reasonable value-add. But it isn’t the whole meal.

When fundamentals are out of whack, nothing else does the work for you. The fanciest Russian nasal spray on Earth will likely be one big expensive disappointment.

Semax is a real molecule with real effects and genuine potential—but it’s a tool, not magic. And if you want access to properly manufactured versions with quality control, make your voice heard at the FDA docket before July 9th.

A new virtual reality program for rehabilitating patients after an ischemic stroke has been developed by specialists at Tyumen State Medical University in Russia. Using this program together with a robotic training device creates the sensation of riding a bicycle for the patient. The results were published in the journal Bulletin of Rehabilitation Medicine of the Russian Ministry of Health.

Ischemic stroke occurs when blood flow to the brain is disrupted: cells that do not receive enough blood begin to die rapidly. After a stroke, patients may develop limb paralysis, speech disorders and other long-term complications, the university explained. Researchers emphasized that stroke is one of the leading causes of disability among adults.

University scientists developed a program designed to restore leg function after an ischemic stroke. The method can be used together with a robotic exercise bike as early as the third day after a patient’s condition stabilizes.

The robotic device turns the pedals on its own while the patient’s feet are fixed in place, and at the same time the person wearing VR goggles sees themselves riding a bicycle. According to the researcher, this therapy proved more effective than standard rehabilitation or recovery using augmented-reality gloves.

In Russia, a drug is being created that can protect against the consequences of stroke, the second most common “killer” of people around the world. It has successfully passed the preclinical trials stage, the drug is ready to be tested on volunteers. Komsomolskaya Pravda spoke with Head of the development team, doctor of chemical sciences, Professor Vadim Negrebetsky at the Russian Academy of Sciences.

BATTLE: CANNOT BE HEALED, BUT CAN BE TREATED

Is it possible to create a kind of “vaccine” against stroke? Yes, risks can be reduced by giving up bad habits. A healthy lifestyle also helps. But is it possible to completely eradicate this disease?

The main difficulty is that many events occur simultaneously in the brain after a stroke. Blood circulation is disturbed, which is accompanied by neurological symptoms, such as speech disorders, limb mobility and swallowing. The blood supply to the brain is noticeably reduced, and this causes severe stress to the body and causes necrosis or premature death of neurons. A stroke can lead to death, but even with a relatively favorable outcome, the consequences can leave a relatively healthy person incapacitated yesterday.

BASED ON A SPACE MEDICINE FROM THE BIN OF THE HOMELAND

Russian scientists turned to a forgotten ancient drug, phenotropil, originally from the USSR, from the 1970s. This substance has a nootropic effect, that is, it can increase attention and concentration, have a beneficial effect on the nervous system’s tolerance to various kinds of loads, and protect interneuronal connections from destruction. It was once used in astronaut training, but it is not convenient to use it for stroke. The drug is in a pill and the “therapeutic window” for stroke, the time when treatment is most effective, is only 6 hours. And how can a person with a stroke take even one pill? During this time, you should still have time to do an MRI or computed tomography with contrast medium. And if you do not have time, the neurons begin to die, the consequences are irreversible.

CHEMICAL BATTERY FOR THE BRAIN

Scientists decided to “spin” the drug to make it better absorbed by the body. At the same time, the chemical formula was also changed, adding a new compound that fights inflammation that starts immediately after a stroke.

The experimental drug has already received the first patients: laboratory mice, rabbits and rats. Their recovery was followed, among other things, with the help of a magnetic resonance tomograph for laboratory animals, by the way, there are only one or two such devices in Russia and countless. And the rodents were tested to see how their ability to enter burrows changed and how animals could swim.

SEIZE, DEMENTIA AND DEPRESSION

Among the additional effects that the drug has, an antidepressant property stands out.

“Now we’re working with different models of depression in the lab, using lab animals at this stage,” says Vadim Gernebetsky. – One of the models is chronic unpredictable mild stress. It most adequately reproduces depression in humans. Another model is associated with processes triggered by inflammation. Our preparation showed good results. And I think the outlook is even broader than stroke and depression help. It also improves performance very well due to the fact that it contributes to the saturation of blood with oxygen. We are also currently testing how the compound works in Parkinson’s disease.

SPECIAL OPINION

“The problem is that only a few nootropics have been shown to be effective, and only for treating certain stroke-related conditions,” he says. Nikolai Kryuchkov, immunologist, candidate of medical sciences, general director of a contract research agency. – In an integrated approach to stroke treatment, these are auxiliary drugs.

Recovery from a stroke first requires an expensive and individually selected system of rehabilitation (recovery of movement, speech, often vision) and subsequent socialization, he emphasizes.

The expert believes that it is far from certain that the successes achieved in animals will be repeated in human trials. There can be many reasons for failure: an error in the selection of the dosage, the mechanism of action may not work in a person and much more.

“It takes a lot of serious research, which is not so easy to fund,” says Nikolai Kryuchkov. – If the drug can be tested in such a way that it can be brought to the domestic market, it will be a great success. But it’s nice that there are developments going on. I would like to believe that Russian development institutions, public and private pharmaceutical companies will find resources for the most promising drugs in this field.

Mechanisms of neurogenesis and angiogenesis in ischaemic stroke: literature review

Abstract

Scientific achievements of recent decades indicate that neurogenesis and angiogenesis are interrelated processes in the struggle for functional recovery after ischaemic stroke. This literature review presents current data on the neurovascular interactions in ischaemic stroke, and describes the role of signalling molecules and growth factors in the regulation of neurogenesis and angiogenesis, which are crucial for neuronal survival and neuroplasticity. The authors conducted a literature search for abnormal neuroblast migration into the ischaemic penumbra and the role of signalling molecules, molecular targets of angiogenesis, and role of endogenous growth factors and neurochemical markers in post-stroke vascular regulation in acute cerebral ischaemia. Relevant keywords were entered into the PubMed and Google Scholar search engines, as well as Scopus, Web of Science, MedLine, The Cochrane Library, EMBASE, Global Health, CyberLeninka, eLibrary, and other databases.

Despite promising results obtained in animal models, and the data from clinical studies, deeper interrelationships between molecular and cellular interactions of neurogenesis and angiogenesis are still not entirely clear. Further study and understanding of complex interactions between neurogenesis and angiogenesis is needed to find targets for exogenous growth factor administration and changes in endogenous molecule expression for treatment of ischaemic brain injury.

Abstract

Some aspects of late and delayed post-stroke rehabilitation of patients with cognitive impairment and various manifestations of brain neuroplasticity are considered. Particular attention is paid to comprehensive rehabilitation programs for post-stroke patients with cognitive impairment, based on the choice of the optimal modes of physiotherapeutic, drug effects and the duration of the course of therapeutic treatment. Similar programs that can be implemented both in a sanatorium and on an outpatient basis, are effective for patients of working age, including in the late recovery period after ischemic stroke. These programs will lead to the restoration of cognitive and motor functions, the normalization of the psychoemotional state of patients.

Effectiveness of a multidisciplinary approach in the rehabilitation treatment of post-stroke patients at the outpatient stage of medical rehabilitation


Abstract


Background. Current approaches to the rehabilitation of stroke patients are based on the early start of rehabilitation measures, a certain sequence, duration, and, of course, the active participation of the patient and their environment. Recently, the scientific medical community has engaged in a serious discussion on the organization of rehabilitation from the perspective of a multidisciplinary approach.
Aim. To assess the impact of the multidisciplinary approach to the outpatient rehabilitation phase on the main health components in stroke patients.
Materials and methods. An experimental, longitudinal, open study included 60 patients with stroke in the late residual period. The studied patients were classified into groups using stratification randomization: Group 1 – 30 (50%) patients with high rehabilitation potential, as well as a high degree of personal interest in undergoing the outpatient phase of medical rehabilitation; Group 2 – 30 (50%) patients who had rehabilitation potential but were, for several reasons, forced to refrain from undergoing the outpatient phase of medical rehabilitation. The duration of the outpatient rehabilitation phase was 14 days: the course of rehabilitation treatment included 10 visits to the rehabilitation department, which operates on the principles of lean manufacturing and a quality management system; 2 days of medical leave; and 2 days of independent work by patients. The multidisciplinary team included a neurologist, physiotherapy physiologist, physical therapy instructor, speech therapist, psychologist, occupational therapist, and a social worker who received special training in medical rehabilitation. As the criteria for assessing the effectiveness of rehabilitation measures during the course of this study, we identified 11 key indicators that allowed us to assess the recovery of neurological deficit, motor functions, possible risk of patients falling, psycho-emotional state, cognitive functions, level of disability, frequency of repeated strokes, number of visits to the clinic place of residence, level of independence, and quality of life of the patient.
Results. When we compared these groups in response to the course of rehabilitation treatment, the patients who passed the outpatient stage achieved a positive result of 91% (a positive response to 10/11 criteria for evaluating effectiveness(Did the effectiveness questions ask about 100% recovery? If not, you didn't ask the correct questions.), while the percentage of response to treatment in the second group was 18% (positive answer for 2/11 criteria for evaluating effectiveness).
Conclusion. To achieve the rehabilitation goals and objectives set for the patient in the late recovery period, it is advisable to conduct the outpatient phase of medical rehabilitation.

Инсульт является одной из самых актуальных медико-социальных и экономических проблем современного общества, продолжая занимать лидирующее место среди всех цереброваскулярных заболеваний по распространенности, смертности и стойкой инвалидизации среди взрослого населения [1, 2]. Современные подходы к реабилитации пациентов, перенесших мозговой инсульт, основаны на раннем начале реабилитационных мероприятий, определенной последовательности, длительности и, конечно, активном участии самого пациента и его окружения [3]. При этом в поздний восстановительный период (6 мес –2 года) инсульта особо возрастает значимость решения задач по поддержанию и совершенствованию когнитивных функций и речи, активному перемещению пациентов с помощью средств дополнительной опоры, совершенствованию ходьбы и навыков самообслуживания [4]. В последнее время в научном медицинском сообществе серьезно обсуждается организация реабилитации с позиции мультидисциплинарного подхода [5, 6]. Объединение специалистов в мультидисциплинарную бригаду, оказывающую помощь в лечении и реабилитации пациентов и работающих как единая команда (бригада) с четкой согласованностью и координированностью действий, открывает новые возможности в реализации задач реабилитации. С этой связи целью настоящего исследования явилась оценка влияния мультидисциплинарного подхода этапа амбулаторной реабилитации на основные компоненты здоровья у пациентов, перенесших инсульт.

Материал и методы

В экспериментальное продольное открытое исследование были включены 60 пациентов с инсультом в позднем восстановительном периоде. Критериями включения в исследование – верифицированный диагноз ишемический инсульт давностью от 6 месяцев до 2-х лет; значения по шкале реабилитационной маршрутизации (ШРМ) 2-3 балла; возможность продуктивного контакта с самим пациентом и его близкими; подписание добровольного информированного согласия на исследование и использование современных методов реабилитации. Критериями исключения из исследования – давность инсульта более 2 лет и менее 6 месяцев; возраст менее 40 и более 80 лет; тяжелая сопутствующая соматическая патология; высокие риски несоблюдения протокола исследования и др.
Исследуемые пациенты были определены в группы методом стратификационной рандомизации: 1 группа – 30(50%) пациентов, имеющих высокий реабилитационный потенциал, а также высокую степень личной заинтересованности прохождения амбулаторного этапа медицинской реабилитации; 2 группа – 30(50%) пациентов, имеющих реабилитационный потенциал, но в силу ряда причин отказавшиеся от прохождения амбулаторного этапа медицинской реабилитации. Достоверные различия групп по возрасту, полу и стадиям заболевания отсутствовали.
Длительность амбулаторного этапа реабилитации составила 14 дней. Курс восстановительного лечения включал в себя 2 дня лечебного отпуска; 2 дня самостоятельной работы пациентов, 10 посещений отделения реабилитации, функционирующего на принципах бережливого производства и системы менеджмента качества. В основе бережливого подхода устанавливалось снижение основных и дополнительных потерь со стороны пациента (прием в одном помещении, снижение трансакционных издержек, организация приема по принципу «точно-вовремя», сокращение ненужной транспортировки медицинских документов) и со стороны медицинских сотрудников отделения (организация рабочего пространства по методу 5S, визуализация данных, внедрения метода всеобщего обслуживания оборудования) [7, 8]. В состав мультидисциплинарной бригады входили прошедший специальную подготовку по вопросам медицинской реабилитации врач-невролог, врач лечебной физкультуры, инструктор лечебной физической культуры, нейропсихолог-логопед, психолог, эрготерапевт, социальный работник. В данном составе бригада разрабатывала индивидуальный план медицинской реабилитации с учетом выраженности неврологического дефицита, а также функциональных нарушений пациента. Для адекватного отражения изменений в состоянии пациента, проделанной медицинским и педагогическим персоналом работы и повышения мотивации к процессу реабилитации использовались клинические шкалы, отражающие патологический процесс, изменения в различных системах. В процессе проведения восстановительного лечения в ежедневном режиме проводилась повторная оценка состояния пациента и корректировался ход восстановительных занятий в зависимости от результатов оценки, что позволило снизить неэффективное использование ресурсов и повысить качество восстановительно лечения.
В качестве критериев для оценки эффективности реабилитационных мероприятий в ходе настоящего исследования были определены 11 основных показателей, позволяющих оценить восстановление неврологического дефицита, моторные функции, возможные риски падения пациентов, психоэмоциональное состояние, когнитивные функции, уровень самостоятельности и качества жизни пациента.
Полученные данные были обработаны статистически в программном пакете Statistica 10.0. Нормальность распределения значений параметров оценивали с использованием критерия Шапиро–Уилка. Данные были представлены в виде среднего арифметического ± стандартное отклонение. Для дальнейших расчетов использовали методы параметрической статистики: для количественных признаков в сравнении групп – критерий Стьюдента, для сравнения показателей внутри одной группы в разные моменты времени – парный критерий Стьюдента. Критический уровень значимости (p-value) при проверке статистических гипотез принимали равным 0,05.

Результаты и обсуждение

Двухнедельная динамика показателей физического компонента здоровья пациентов, перенесших инсульт, оказалась более выраженной в четырех из пяти анализируемых показателей у пациентов, которые находились на амбулаторной реабилитации (табл. 1).

ШРМ (шкала реабилитационной маршрутизации) чувствительна для оценки реабилитационного потенциала пациентов, перенёсших инсульт. При сравнении эффективности восстановительного лечения при исходно одинаковых значениях в обеих группах (2-3 балла) только пациенты с амбулаторным этапом реабилитации показали статистически значимую положительную динамику (13,0%) снижения значений ШРМ. Быстрое снижение неврологического дефицита, оценённое по ШРМ, в данной группе способствует в дальнейшем более благоприятному исходу инсульта.
Уровень мобильности, оцененный по индексу мобильности Ривермид, статистически значимо увеличился (p<0,01) на 4,3% у пациентов 1 группы. В то же время пациенты 2 группы по способности перемещать собственное тело не продемонстрировали динамику через 2 недели (p>0,05).
Степень нарушенных функций и самостоятельности в повседневной жизни при исходно одинаковых значениях (p>0,05) в обеих группах статистически значимо (p<0,01) снизилась на 18,4% у пациентов, прошедших двухнедельную амбулаторную реабилитацию. При этом значение шкалы Рэнкин во 2 группе снизилось только на 5,3%, что оказалось статистически не значимо (p>0,05).
Болевой синдром статистически значимо (p<0,05) уменьшился в обеих группах вне зависимости от формы проводимого восстановительного лечения. При этом значения Визуальной аналоговой шкалы (ВАШ) в обеих группах в динамики оказались сопоставимы (p>0,05). Полученные результаты коррелируют с многочисленными литературными данными [3, 9, 10]. Большинство отечественных и зарубежных исследователей данной проблемы отмечают снижение болевого синдрома у пациентов, перенёсших инсульт, в позднем восстановительном периоде независимо от лечебной и реабилитационной тактики, в том числе и в случаях ее полного отсутствия. Вероятнее всего, в основе данного эффекта лежит активация естественных собственных адаптационных и восстановительных механизмов организма человека [9].
У пациентов 2 группы, по сравнению с группой на амбулаторной реабилитации, установлены более частые случаи травматизма, вызванные частыми падениями пациентов как исходно (+14,6%), так и в процессе двухнедельного наблюдения (+14,6%). При опросе самих пациентов и/или их близких установлено, что у 39% (12/30 человек) отмечались ежедневные падения с высоты собственного роста. В то же время у пациентов 1 группы указанный показатель был значительно ниже – 20% (6/30 человек). Данное осложнение инсульта влияет как на высокий показатель травматизма пациентов 2 группы, так и на выбор тактики восстановительного лечения. 1% (3/10 человек) пациентов 2 группы в течение позднего восстановительного периода имели серьезные повреждения: перелом бедра на стороне пареза – 1 человек, перелом таза – 1 человек, перелом запястья – 1 человек. В 1 группе риск падений был ниже, серьезных травм, полученных при падениях, зафиксировано не было, это могло опосредованно повлиять на активное желание посещения отделения медицинской реабилитации.
Двухнедельная динамика показателей психического компонента здоровья пациентов, перенесших инсульт, оказалась более выраженной по одному из трех анализируемых показателей у пациентов, которые находились на амбулаторной реабилитации (табл. 2).

При оценке когнитивных нарушений в обеих группах при одинаково низком исходном значении по Монреальской шкале, указывающей на снижение когнитивных функций (внимание и концентрация, исполнительные функции, память, речь, абстрактное мышление, счет и ориентация), в 1 группе отмечена положительная динамика на фоне восстановительного лечения (+3,5%, p<0,01), в то время как во 2 группе динамика анализируемого показателя оказалась статистически не значимой (p>0,05).
Исходно уровень тревожных расстройств пациентов обеих групп соответствовал нормативному показателю и не превышал 7 баллов по Госпитальной школе тревоги и депрессии. Через 2 недели как в 1 группе, так и во 2 группе отмечено снижение показателя тревожности в 1,1 раза (p<0,05). Наличие или отсутствие амбулаторного этапа реабилитации не повлияло на итоговые значения уровня тревоги. Вероятнее всего, подобная динамика объясняется психологической адаптацией к сложившейся ситуации пациентов; завершением острой стадии стрессовой ситуации, вызванной основным заболеванием; принятием собственной личности и окружающего пространства в сложившейся новой реальности необходимости существования с проявлением заболевания, его осложнений и последствий.
В то же время наличие амбулаторного этапа восстановительного лечения, включающего в свой арсенал индивидуальные и групповые занятия с медицинским психологом, отразилось в статистически значимом (p<0,05) снижении уровня депрессии на 10,6% по сравнению с исходными данными. Пациенты 2 группы не показали динамики результатов (p>0,05). При этом динамическое наблюдение данной группы показало увеличение коэффициента вариации показателей с 21% до 30%, что свидетельствует о расширении разброса значений данного признака спустя 2 недели.
Социальная дезадаптация человека является одной из острейших проблем реабилитации постинсультных больных как для системы здравоохранения, так и для общества, в целом. Двухнедельная динамика показателей социального компонента здоровья пациентов, перенесших инсульт, оказалась более выраженной во всех анализируемых показателях у пациентов, которые находились на амбулаторной реабилитации (табл. 3).

Степень зависимости пациентов от близких и родственников оказалась на 22% выше во 2 группе (p<0,01) в сравнении с группой прошедших амбулаторный этап. При долгосрочной оценке через 6 месяцев показатель самостоятельности в группе пациентов, прошедших амбулаторный этап в сравнении с пациентами 2 группы показал статистически значимую разницу (p<0,05). Степень зависимости у пациентов 2 группы статистически значимо возросла (p<0,05). Это дает право предположить, что амбулаторный этап медицинской реабилитации способствует формированию самостоятельности пациентов и снижает степень зависимости от посторонних в повседневной жизни.
При исходно одинаковом уровне социальной активности спустя две недели с момента наблюдения у пациентов 1 группы определялось статистически значимое (p<0,05) повышение уровня потребности межличностного общения, участия в социальных процессах и проявлении интересов. В то время как у 13% (4/30) пациентов 2 группы по результатам опроса близких, напротив, была отмечена тенденция к снижению социализации, уровня вербального общения, реакции на окружающие изменения. В процессе долгосрочного наблюдения 17%(5/30) пациентов 2 группы на фоне отрицания проблем начали злоупотреблять алкоголем (более 8 литров алкоголя в год и 3-4 дня полной трезвости в неделю).
Показатель уровня приверженности к лечению у пациентов 1 группы оказался выше (p<0,01), чем у пациентов 2 группы, что соответствует распределению пациентов по анализируемым группам – одним из стратификационных критериев рандомизации стало согласие пациента проходить амбулаторный этап реабилитации. Стоит также отметить, что прохождение амбулаторного этапа, и, по всей видимости, положительный эффект от реабилитации, статистически значимо (p<0,05) повышает комплаентность пациентов к лечению в будущем: пациенты начинают понимать основные цели реабилитационного процесса, появляется интерес к достижению еще большего реабилитационного потенциала, новых результатов.
Амбулаторной этап реабилитации направлен на стабилизацию течения основного сосудистого заболевания, адаптацию к повседневным бытовым нагрузкам, уменьшению выраженности двигательных и других постинсультных нарушений. К задачам мультидисциплинарной бригады в рамках отделения реабилитации МО относится выработка со стороны пациентов заместительных компенсаций, приспособлении к дефекту, ликвидации артралгий, коррекции психопатологических проявлений, полном или частичном восстановлении бытовой активности.

Заключение

Наличие амбулаторного этапа восстановительного лечения положительно сказывается на состоянии физического, психического и социального компонента здоровья пациентов, перенесших инсульт. В то же время, даже отсутствие амбулаторной реабилитации характеризуется постепенным снижением уровня боли и тревоги, что можно объяснить активизацией собственных адаптационных механизмов организма человека.
Для достижения реабилитационных целей и задач, поставленных перед пациентом в позднем восстановительном периоде, целесообразно последовательное проведение амбулаторного этапа медицинской реабилитации. При данном методе восстановительного лечения положительный эффект отмечается на 91% (10/11) основных критериев оценки, не оказывая влияния, в краткосрочный период, на зависимость пациента от близких и родственников.
В условиях непрерывной оптимизации и усовершенствования системы здравоохранения возникает потребность в использовании систем реабилитации с целью обеспечения успешной и непрерывной реабилитации на амбулаторном этапе с учетом места нахождения больного, а также возможностей перемещения пациента к месту занятий. Подобные методы позволят обеспечить персонифицированный непрерывный процесс реабилитационного лечения, способствующий достижению максимально возможного уровня самостоятельности пациента, а также сохранить уже достигнутые результаты восстановительного лечения. Одной из таких систем может стать дистанционный этап медицинской реабилитации.

Abstract: This paper discusses the mechanisms responsible for the process (neurogenesis, neuroplasticity, and readaptation) involved in the recovery of neurological functions following ischemic stroke (IS) and possible approaches to the drug support of cerebral reparative processes. It presents data on the rate and etiology of poststroke cognitive impairments (CI) that are one of the important factors that negatively influence the outcome of rehabilitation measures. The results obtained from a recent study of the efficacy of the neurometabolic agent citicoline in the recovery period of IS are given. The use of citicoline is shown to cause a significant regression of CI and to improve quality of life in patients who have sustained IS for the first time.

Abstract: This paper presents the January 2016 results of the randomized placebo-controlled multicenter study Cerebrolysin and Recovery After Stroke (CARS), which demonstrates the positive effect of cerebrolysin versus placebo according to the primary efficiency criterion, the Action Research Arm Test (ARAT) scale, and total outcome 90 days after disease onset. Ischemic stroke (IS) is characterized by high prevalence, mortality, and disability rates. Therapy aimed to correct one biochemical or molecular stage of ischemic cell injury fails to treat stroke, suggesting that it is necessary to study multimodality therapy affecting several related pathophysiological components. The CARS investigation enrolled mainly patients with moderate or severe IS (the mean National Institutes of Health Stroke Scale score was 9 at baseline). The specific features of the CARS study versus those of other clinical trials of neuroprotectors were the initial planning of narrower end criteria of efficiency (arm motor function recovery, whereas the major goal of many investigations was to reduce mortality rates), as well as a standardized rehabilitation program in both treatment groups. Such investigations did not previously consider the nature and volume of rehabilitation measures, although the latter may have a substantial impact on the outcome of stroke. The CARS study is the first among the previously conducted clinical trials of neuroprotectors that has attained the primary objective (to restore motor function), which presents fresh opportunities for the medical support of rehabilitation measures in patients with IS.
