In this issue of Neurology®, using a national sample from the Veterans Health Administration, Tarko et al.¹ provide 30-day and longer mortality estimates for non-Hispanic White, non-Hispanic Black, and Hispanic patients hospitalized with incident stroke and examine the association of race and ethnicity with stroke mortality.

Paying it forward

 I got slightly criticized for providing questions for your doctor on the first day. While true that lots of stroke patients are not mentally capable of understanding what occurred to them in the first days. The whole point is to ask your doctor, therapists and hospital what they are doing in the first days to get you recovered. Even if you can't ask that question immediately, it should be asked and demand an answer so future stroke patients will have a better chance of recovery than you had.  If WE don't hold the stroke medical world's feet to the fire NOTHING WILL EVER GET BETTER!

What to do the second day after your stroke

You tell your doctor they have killed off
In each minute,
1.9 million neurons die
14 billion synapses die
12 km (7.5 miles) of myelinated fibers die
brain ages 3.6 years each hour without treatment
(This probably can be cut in half after the original emergency is over, this is the continuing damage from the neuronal cascade of death.)
Followup with the first day questions they still haven't answered.
What are they going to do to stop those neuronal deaths from progressing? Charge them $1 for every neuron they fail to save($950,000 a minute).  This is probably the only thing that will concentrate your doctors mind and the hospital they work for. Maybe, just maybe that might be the tipping point to get them to solve the neuronal cascade of death. Nothing else seems to work.
It should be fairly easy to get an app for your iPad or iPhone that keeps a running total of dead neurons that can be shown every time your doctor does rounds. All those interns following may understand why neurologists aren't respected. 

What to do the first day after your stroke

You are going to have to be very demanding about this. Don't take non-specific comments for answers. This is your life and health depending on the answers.

12 simple questions for your doctor to respond to. No specific answers, you don't even have a functioning stroke rehab doctor.

1. First question - What exactly are you doing to stop the neuronal cascade of death in the first week? If there is no definite answer or they don't even know what it is, Scream in their faces that they are assisting in killing your neurons.  Millions every minute.

2. Which of these 31 possibilities are they doing the first week?

3.Demand to know what the stroke rehab protocols are and their efficacy. Do not back down they should know all this by heart. If they don't they are incompetent. If they haven't been measuring previous patients recovery they are just acting as a doctor, get a new one.

4. How are you planning on getting me to 100% recovery?

5. How are you planning on getting me to 100% recovery?

6. How are you planning on getting me to 100% recovery?

7. How are you planning on getting me to 100% recovery?

8. How are you planning on getting me to 100% recovery?

9. How are you planning on getting me to 100% recovery?

10. How are you planning on getting me to 100% recovery?

ad infinitum until a proper answer is given

11. If you hear 'All strokes are different, all stroke recoveries are different', that is a sure sign of laziness, call them on it, ask for research article proof. This statement is proof your doctor knows nothing about getting you to full recovery.

12. Ask when they are going to tell you all this stuff.
