My blog is not to help survivors recover, it is to have the 10 million yearly stroke survivors light fires underneath their doctors, stroke hospitals and stroke researchers to get stroke solved. 100% recovery. The stroke medical world is completely failing at that goal, they don't even have it as a goal. Shortly after getting out of the hospital and getting NO information on the process or protocols of stroke rehabilitation and recovery I started searching on the internet and found that no other survivor received useful information. This is an attempt to cover all stroke rehabilitation information that should be readily available to survivors so they can talk with informed knowledge to their medical staff. It lays out what needs to be done to get stroke survivors closer to 100% recovery. It's quite disgusting that this information is not available from every stroke association and doctors group.

TeamStudy app supports NFL players health research - Why not stroke?

A novel idea that will never be followed up by our fucking failures of stroke associations. A similar smartphone app for stroke should easily be able to be created. I can't see what it has since I have an android instead.

The Football Players Health Study at Harvard University – a series of studies that examine the health and well–being of former football players and aims to develop more effective diagnostic and therapeutic interventions – launched TeamStudy, an iPhone research app built using ResearchKit, the open source software framework designed by Apple. Former NFL players were integral in designing the app to focus on health issues that matter most to them, such as memory, balance, heart health, pain and mobility. For the first time, anyone with an iPhone can support the study of athlete health and help researchers better understand the health issues that matter most to former NFL players. TeamStudy is available as a free download from the App Store.

So much than can be done in stroke yet no one lifts a finger to actually solve survivor problems.

Report calls use of prompts “a promising strategy”.
A pilot study finds that using smartphone reminders to prompt people to get moving may help reduce sedentary behavior. The study was supported by the American Cancer Society, with technical expertise provided by the e–Health Technology Program at the MD Anderson Cancer Center. It appears in the Journal of Medical Internet Research. Evidence has linked sedentary time to increased risk of breast, colorectal, ovarian, endometrial, and prostate cancers as well as weight gain, higher BMI, and obesity. Nevertheless, adults in the U.S. spend an average of about 8 waking hours per day being sedentary. Participants wore accelerometers, to measure movement, and carried smartphones for seven consecutive days. Participants who reported more than two hours of sitting during the previous day or replied that they were sitting during any random smartphone assessment received a message emphasizing that long uninterrupted sitting is bad for health, and encouraging them to stand up and move around more, and to sit less. Over the seven–day study period, participants had significantly fewer minutes of daily sedentary time and more daily minutes of active time than controls. Accelerometers recorded three percent less sedentary time than control participants, equaling about 25 minutes of time spent engaged in activity rather than in sedentary behavior on any given day. Due to the pilot nature of the study it had inherent limitations that should be noted: it was not randomized and the duration was brief. Nonetheless, the authors say: “Overall, simple smartphone prompts appear to be a promising strategy for reducing sedentary behavior and increasing activity, though adequately–powered and well–designed studies will be needed to confirm these preliminary findings. “

Game doubles as stroke app using iPhone’s Force Touch & robotics to deliver rehab

All these positive statements but yet I never see any publicly published protocols on any of these. Because we have NO stroke leadership or strategy anywhere in stroke. If we had a great stroke association we would have cutting edge interviews asking where are the protocols and when will it be delivered to stroke departments?  This won't occur in your lifetime, your childrens' lifetime or even your grandchildrens' lifetime. We are all fucking screwed basically forever.

I still prefer the cockroach stomping game.


Game doubles as stroke app using iPhone’s Force Touch & robotics to deliver rehab

A team of physicians, developers, artists, and programmers are developing and testing a gaming app, Bandit’s Shark Showdown, to both improve recovery in stroke patients and maybe even make it fun.
There are a lot of stroke apps out there, from aids to help patients recognizing potential stroke signs to decision support tools for diagnosis. Featured in a recent profile in The New Yorker, neurologist Dr. John Krakauer expressed his frustration with the limitations of traditional post-stroke rehabilitation and hopes to unlock the potential of robotic therapy combined with cutting-edge interactive entertainment. He has helped found the “Brain, Learning, Animation, Movement lab”(BLAM), a “Pixar-grade” team of engineers, designers, artists and programmers that work with clinicians to use the best of art and science further research on the brain.
Bandits Shark Showdown
One of their studies involves investigating motor recovery as part of stroke rehabilitation. This is done through utilizing a combination of the game, Bandit’s Shark Showdown, with a robotic arm, the Hocoma ArmeoPower, to encourage intensive game-based therapy. Instead of traditional rehabilitation where the focus is often task-based, the focus here is simply on movement in a virtual environment, directing a dolphin through various challenges like battling dangerous sharks and other enemies.
Three members of BLAM, who also own an indie gaming company called Max and Haley, designed Bandit’s Shark Showdown. In fact, it wasn’t really designed as a medical app. Rather, they had already been in the business of designing apps based on animal movements and were working with the National Aquarium in Baltimore to design a game based on dolphin movements.
They’re also testing the use of Force Touch in the game according to an interview in the Verge. Players (patients recovering from strokes) have to apply varying levels of pressure to control their dolphin as it goes after sharks. They hope that this can help users work on balancing force and dexterity, offering yet another tool for recovery.
After a cerebral vascular accident, several mechanisms have been traditionally postulated to allow to motor recovery. These include neural plasticity with periinfact reorganization (undamaged regions of the brain taking over for damaged regions), diaschisis (regions remote from infarcted location are initially deactivated but later recover), and ipsilateral/contralateral hemisphere activity (inhibition and excitability) 1. The majority of motor recovery after a stroke is thought to occur within the first 3 months. This is an ideal time for Acute Rehabilitation, where a team consisting of a rehabilitation physician, therapists (physical, occupational, and speech), neuropsychologist, nursing, social workers and more attempt to maximize functional outcomes for post-stroke survivors.
Current approaches to therapy during this time include increasing attention to the affected paretic region, while in the past having focused on compensatory strategies. Techniques such as Constraint-induced movement therapy (CIMT) have been utilized, supported by recently published clinical trials.
However, as noted by Dr. Krakauer, the ability to provide enough high-intensity and frequency of such therapy may be limited in some post-stroke care environments. Apps offer an opportunity here and while there are many available, the quality of these stroke apps has been found to be lacking. Robotics offer an intriguing avenue to increase the frequency of such therapy and combining this with an immersive game, such as Bandit’s Shark Showdown, may offer even more benefits. The upcoming clinical trial of this technology through BLAM, with stroke patients at Columbia University and a clinic in Zurich, is certainly exciting and is one to follow for all interested in neuro-rehabilitation.

Can a 3D Touch app help stroke rehab patients recover dexterity and strength in their hands?

We'll never know because we have NO stroke strategy or stroke leadership in any part of stroke. A great stroke association would be working this like mad to create rehab applications to be sold to survivors and use the proceeds to fund stroke research. But we don't have anyone in stroke with enough brains to do something so fuckingly simple. And the boards of directors must be ok with such incompetency.

Piano tiles game for finger rehabilitation

Don't tap the white tile as it slides by. I'll have to watch this quite a few times for action observation before I even attempt this. It is almost too fast for my good hand. I currently have no individual finger movement on my left hand.
Piano Tiles is a single-player mobile game launched on March 28, 2014 by Umoni Studio, specifically by creator Hu Wen Zeng.

An Acute Stroke Evaluation App A Practice Improvement Project

Well shit, something minimally effective. Now if we could just get our stroke medical teams to solve all the f*cking problems in stroke and create stroke rehabilitation protocols then maybe they might be worth paying. This would means thousands of hospitals trying to come up with something effective and useful as compared to having a great stroke association solve this for everyone worldwide. But we don't have anything close to a great stroke association.  You'll have to deal with your post-stroke disabilities  for the next 50 years because nothing useful will come before that under the current leadership.

Abstract

A point-of-care workflow checklist in the form of an iOS (iPhone Operating System) app for use by stroke providers was introduced with the objective of standardizing acute stroke evaluation and documentation at 2 affiliated academic medical centers. Providers used the app in unselected, consecutive patients undergoing acute stroke evaluation in an emergency department or hospital setting between August 2012 and January 2013 and August 2013 and February 2014. Satisfaction surveys were prospectively collected pre- and postintervention from residents, staff neurologists, and clinical data specialists. Residents (20 preintervention and 16 postintervention), staff neurologists (6 pre and 5 post), and clinical data specialists (4 pre and 4 post) participated in this study. All 16 (100%) residents had increased satisfaction with their ability to perform an acute stroke evaluation postintervention but only 9 (56%) of 16 felt the app was more help than hindrance. Historical controls aligned with preintervention results. Staff neurologists conveyed increased satisfaction with resident presentations and decision making when compared to preintervention surveys. Stroke clinical data specialists estimated a 50% decrease in data abstraction when the app data were used in the clinical note. Concomitant effect on door-to-needle (DTN) time at 1 site, although not a primary study measure, was also evaluated. At that 1 center, the mean DTN time decreased by 16 minutes when compared to the corresponding months from the year prior. The point-of-care acute stroke workflow checklist app may assist trainees in presenting findings in a standardized manner and reduce data abstraction time. The app may help reduce DTN time, but this requires further study.

Researchers from the Technical Research Centre for Dependency Care and Autonomous Living (CETpD), on the UPC’s Vilanova i la Geltrú Campus, have designed a device that detects falls in elderly people without them having to press a button for assistance. It pinpoints their location inside or outside the home and notifies the emergency services if necessary.
The Universitat Politècnica de Catalunya · BarcelonaTech (UPC) leads the consortium behind the European project Fall Detector for the Elderly (FATE), which also includes TicSalut, 061 CatSalut Respon, Hospital Clínic and EAP Sardenya. The fall detector, which is currently being trialled, brings greater safety both inside and outside the home.
The FATE system consists of a small, highly sensitive fall detector fitted into a belt that users wear throughout the day. The device’s sensors detect movements consistent with a fall and integrated wireless technology locates the user’s position, inside or outside the home.
The system also includes a bed presence sensor that detects prolonged absences during the night and triggers an alarm if the user does not return to bed within a stipulated period. The aim is to minimise the effects of an accident on fragile patients who may not be physically capable of calling for help if they suffer a fall.
The technology for the FATE device was developed by researchers at the Technical Research Centre for Dependency Care and Autonomous Living (CETpD) on the UPC’s Vilanova i la Geltrú Campus, under the direction of Joan Cabestany.
Direct contact with emergency services
When the FATE device detects that a user has suffered a fall, it sends an automatic notification to the Catalan ambulance service, 061 CatSalut Respon, which locates the patient and contacts them to establish the details of the accident. The situation can then be assessed by the CatSalut Respon medical team, who provide guidance on how best to deal with the situation and send an SEM ambulance if further medical assistance is required.
The FATE protocol greatly improves patient safety, primarily for those who live alone, as it provides the security of automatic medical attention in the event of a fall.
FATE trialled in elderly patients
Patients selected for the trial by the project team in Catalonia were taken from the primary care group at EAP Sardenya and Hospital Clínic in Barcelona.
The study is divided into two six-month periods: during the first, half of the patients wear the device and the other half serve as a control group; during the second, the groups are reversed.
Over the course of the study patients are visited on a monthly basis and contacted by telephone each week to evaluate their general state of health and to discuss any problems with the device.
The European project is coordinated by the UPC and carried out by a consortium that also includes partners in Ireland and Italy. The study population for the pilot phase comprises 50 patients in Ireland, 80 in Italy and 75 in Catalonia and was selected according to the relative risk of suffering an accidental fall or a fall caused by an existing medical condition.
Falls are a serious problem in the elderly population, estimated to affect 30% of over-65s and often causing severe physical restrictions. The worst effects are generally observed in those living alone who, in the event of a fall, may be unable to move and can spend considerable periods of time on the ground before they receive medical assistance. The delay in treatment can lead to irreversible health problems.
Immediate detection enables an immediate response from the 061 emergency services, ensuring that the necessary medical assistance is provided as swiftly as possible.
The FATE project began in 2012 and is scheduled to run for 39 months. The pilot phase started at the beginning of 2014 and will continue into 2015.

New app designed to monitor stroke risk

Stroke researchers and technology developers have teamed up to design a new phone application which they hope will help save lives.
Specialists from AUT are the brains behind a new app which they say can help detect a person's stroke risk.
Users can calculate their risk by simply entering their age, blood pressure reading and dietary habits into the free Stroke Riskometer.
Professor Valery Feigin said the app is very simple to use.

If I were to have taken that test 7 years ago the risk would have been nil.

The American Heart Association is teaming up with researchers from the University of California-San Francisco to create a digitally based research network to study heart disease and stroke.
The Health eHeart Study will  use technology, such as smartphones and computers, to track people’s health and habits over many years.. The research is expected to provide information on what causes cardiovascular disease and how to fight it.
The study has been positioned as a “digital Framingham” in the footsteps of the renowned Framingham Heart Study, which launched in 1948 and continues to follow cardiovascular development over decades in three generations of participants.
Researchers hope to sign up as many as 1 million people to participate. Anyone can join the study, which is being featured in a CNBC documentary airing tonight at 9 p.m. EDT.

Emergency Medical Dispatch Captain Philip Weiss reportedly wrote a letter to the FDNY asking that the dispatcher, EMT Joann Hilman-Payne, and her colleagues be recognized for having gone above and beyond in rescuing Mary Thomas.
“Throughout the entirety, [Hilman-Payne] worked to keep the patient awake, she never lost her own composure and remained calm while attempting to elicit more information from the patient,” Weiss wrote.
Thomas was reportedly working as a housekeeper at the apartment where she had a stroke, which is part of what made locating her so difficult.
Hilman-Payne reportedly stayed on the phone with her to keep her awake, though she was unable to understand much of what Thomas was saying, as her speech was badly slurred.
Efforts were made by the FDNY, the NYPD, and Verizon to trace Thomas’ cell phone, but success only came around 8:30 p.m. when Verizon was able to successfully pinpoint Thomas’ cell signal at an East 72nd Street apartment.
Thomas was rushed to Lenox Hill Hospital, and is reportedly being treated in the Intensive Care Unit.

Abstract: Article describes Bangaten, a new version of Banga, a smart phone application that uses computer-presented images to support naming (word finding) practice, a form of therapy for people with aphasia. Banga combines delivery on easily portable smart phone devices with remote management, allowing a therapist to monitor a patient’s practice, and change the materials on a patient’s device, without requiring office visits. Early user experience shows that Bangaten offers useful cross-platform operation, on both Android and iPhone devices, including remote management of a client's device. The Use of Smartphone Applications in Stroke Rehabilitation in Korea

Objective We studied the use of smartphone technology in stroke rehabilitation in Korea and gathered opinions on how it would best be utilized it in a clinical setting.
Method Physiatrists, occupational therapists, physical therapists, and rehabilitation ward nurses were surveyed to examine smartphone propagation among the rehabilitation team, current therapeutic knowledge, the use of smartphone technology, and perceptions regarding the potential therapeutic use of smartphones in rehabilitation. The respondents were also asked to specify the applications considered to be the most appropriate for rehabilitation. We also examined applications available for stroke rehabilitation at Android and Apple stores.
Results Of the respondents, 92% had never using smartphone technology in rehabilitation with their clients. The greatest barrier to use was that "smartphone technology and appropriate applications were not available for rehabilitation settings" (71.4%). Areas identified as most appropriate for smartphone use in therapy included provision of information (82.4%) and cognitive (72.5%) and language training (68.1%). We found only a few applications in android and Apple application stores. Of the respondents, 89% intended to use smartphone applications in rehabilitation in the future.
Conclusion Smartphone applications developed for stroke rehabilitation are promising. Further research and the development of new therapeutic applications for use in rehabilitation and across health care are needed.
