Changing stroke rehab and research worldwide now.Time is Brain! trillions and trillions of neurons that DIE each day because there are NO effective hyperacute therapies besides tPA(only 12% effective). I have 523 posts on hyperacute therapy, enough for researchers to spend decades proving them out. These are my personal ideas and blog on stroke rehabilitation and stroke research. Do not attempt any of these without checking with your medical provider. Unless you join me in agitating, when you need these therapies they won't be there.

What this blog is for:

My blog is not to help survivors recover, it is to have the 10 million yearly stroke survivors light fires underneath their doctors, stroke hospitals and stroke researchers to get stroke solved. 100% recovery. The stroke medical world is completely failing at that goal, they don't even have it as a goal. Shortly after getting out of the hospital and getting NO information on the process or protocols of stroke rehabilitation and recovery I started searching on the internet and found that no other survivor received useful information. This is an attempt to cover all stroke rehabilitation information that should be readily available to survivors so they can talk with informed knowledge to their medical staff. It lays out what needs to be done to get stroke survivors closer to 100% recovery. It's quite disgusting that this information is not available from every stroke association and doctors group.

Freeman Health System receives ‘Get With the Guidelines' awards for heart, stroke care

Only a failed hospital would tout 'care' awards rather than telling us RECOVERY RESULTS!

Tell me EXACTLY where I'm wrong with that analysis! oc1dean@gmail.com Don't bother to reply anonymously if you can't stand by your name and position.

Freeman Health System receives ‘Get With the Guidelines' awards for heart, stroke care

Freeman Health System recently received three "Get With the Guidelines" achievement awards from the American Heart Association and American Stroke Association.

"The receipt of these accolades is a testament to the dedication and commitment of every team across Freeman Health System. I am so proud of the work our teams do every day," Freeman Health System President and CEO Matt Fry said in a statement. "Their focus on providing high-quality, contemporary healthcare is truly inspiring. It is an honor to serve our community, and our entire team is committed to continuing our pursuit of expanding access, improving quality, enhancing the patient experience and reducing costs."

Freeman received the following achievement awards:

• Coronary Artery Disease STEMI Receiving Center, Gold Plus with Target: Type 2 Diabetes Honor Roll.

• Coronary Artery Disease NSTEMI, Gold with Target: Type 2 Diabetes Honor Roll.

• Stroke Gold Plus with Target: Type 2 Diabetes Honor Roll.

"It is such an honor for Freeman Health System to be recognized for these awards," Dr. Matthew Grounds, stroke medical director, and Kelley King, RN and accreditation coordinator, said in a statement. "This demonstrates Freeman's commitment to following best practice guidelines in caring for stroke patients. Great leadership and teamwork make this possible, and we are proud of this."

According to Freeman, each year, more than 1.2 million people in the U.S. are hospitalized with acute coronary syndrome, according to the American Heart Association. About 30% of ACS cases are classified as STEMI, caused by a complete blockage in a coronary artery. Stroke is the fourth-leading cause of death and a leading cause of disability in the U.S., according to the American Heart Association.

Since 2019, Freeman Health System has earned 12 "Get With the Guidelines" achievement awards, including five in 2025.(So, a COMPLETE FUCKING FAILURE FOR 6 YEARS ALREADY! Going for a record?)

MMH recognized for its commitment to excellence in cardiovascular care for 6 years in a row

 Two absolute stupidities here:

  1. Stroke has been called neurological disease by the WHO since 2006 not cardiovascular, once again proving our medical 'professionals' don't keep up to date in their field! Why would your trust anything coming out of such incompetence?
  2. And 'care'; Survivors don't care about 'care; THEY WANT 100% RECOVERY! ARE YOU THAT FUCKING STUPID?

MMH recognized for its commitment to excellence in cardiovascular care for 6 years in a row

  • Midland Memorial Hospital has received two American Heart Association Get With The Guidelines achievement awards for following evidence-based treatment guidelines to improve patient outcomes and reduce hospital readmissions.

Midland Memorial Hospital (MMH) has received two American Heart Association Get With The Guidelines achievement awards for demonstrating a commitment to following up-to-date, research-based guidelines for the treatment of heart disease and stroke, helping save lives, support recovery and reduce hospital readmissions.

Heart disease and stroke are the No. 1 and No. 4 causes of death in the United States, respectively, according to the American Heart Association's 2026 Heart Disease and Stroke Statistics Report.

Studies show patients can experience better outcomes when care(NOT RECOVERY!) teams consistently follow evidence-based treatment guidelines.

Get With The Guidelines puts the expertise of the American Heart Association and American Stroke Association to work for hospitals nationwide, helping ensure patient care(NOT RECOVERY!) is aligned with the latest science-backed guidelines. As a participant in the Get With The Guidelines programs, MMH qualified for the awards by demonstrating a sustained commitment to improving the quality of cardiovascular care(NOT RECOVERY!).

"MMH is committed to delivering high-quality care(NOT RECOVERY!) by adhering to the latest treatment guidelines and streamlining processes to ensure timely, appropriate care(NOT RECOVERY!) for heart attacks and strokes," Stephen Bowerman, president/chief executive officer, said in a news release. "Get With The Guidelines programs help our teams put proven science into practice every day, supporting better outcomes so more people in the greater Midland area can live longer, healthier lives."

This year, Midland Memorial Hospital received these achievement awards:

* Get With The Guidelines-Coronary Artery Disease STEMI Receiving Gold with Target: Type 2 Diabetes

* Get With The Guidelines-Coronary Artery Disease NSTEMI Gold with Target: Type 2 Diabetes

"These awards reflect MMH's commitment to caring for people in their community who need cardiovascular care(NOT RECOVERY!)," said Karen E. Joynt Maddox, M.D., MPH, chair of the American Heart Association Quality Oversight Committee. "By following the American Heart Association's quality improvement protocols, MMH can help advance our shared vision of better patient outcomes, fewer readmissions and lower mortality rates — a win for patients, families and health care(NOT RECOVERY!) systems.

SIH Herrin and Memorial Hospitals Earn National Stroke Care Award

This is the whole problem in stroke enumerated in one word; 'care'; NOT RECOVERY!

Our non-existent stroke leadership should be demanding RECOVERY NOT 'CARE'! This is for you to solve: WSO!

My god, anyone in the business world would be fired immediately for managing or caring about something rather than delivering RESULTS. And this is why this is a complete fucking failure! This does nothing to guarantee recovery for survivors!

If your stroke medical 'professional'/hospital is touting 'care' it means they are a failure because they are delivering 'care'; NOT RECOVERY! I would never go to a failed hospital! Anytime I see the word 'care' associated with a stroke hospital; I immediately think fucking failure!

YOU have to get involved and change this failure mindset of 'care' to 100% RECOVERY! Survivors want RECOVERY, NOT 'CARE'!

I see nothing here that states going for 100% recovery! You need to create EXACT PROTOCOLS FOR THAT!

ASK SURVIVORS WHAT THEY WANT, THEY'LL NEVER RESPOND 'CARE'! This tyranny of low expectations has to be completely rooted out of any stroke conversation! I wouldn't go there because of such incompetency as not having 100% recovery protocols!

RECOVERY IS THE ONLY GOAL IN STROKE!

GET THERE!

SIH Herrin and Memorial Hospitals Earn National Stroke Care Award

CARBONDALE, Ill. (WSIL) — SIH Herrin Hospital and SIH Memorial Hospital of Carbondale have earned national recognition for their commitment to providing fast, evidence-based stroke treatment that improves patient outcomes.

Both hospitals recently received the Get With The Guidelines® – Stroke Gold Plus with Target: Stroke Honor Roll award from the American Heart Association and the American Stroke Association. The designation highlights hospitals that consistently deliver high-quality care(NOT RECOVERY!) designed to reduce death and disability caused by stroke.

To qualify for the award, hospitals must meet strict performance standards aimed at shortening the time between a stroke patient’s arrival at the hospital and the start of life-saving treatment, including clot-busting medications.

Stroke remains the fourth leading cause of death in the United States and a major cause of long-term disability. The condition occurs when blood flow to the brain is blocked or a blood vessel ruptures, depriving brain tissue of oxygen and nutrients. Quick treatment is essential to minimizing damage and improving recovery.

“Rapid treatment truly changes what recovery looks like for stroke patients,” said Andrea Loggini, MD, Neurointensivist and Neuroscience Inpatient Medical Director at Southern Illinois Healthcare. “This recognition reflects how consistently our teams follow national guidelines and, just as importantly, how well they work together to deliver time-sensitive treatment that can help patients regain speech, movement and independence.”

The Get With The Guidelines® program is designed to help hospitals align stroke care(NOT RECOVERY!) with the latest medical research. It promotes early detection and standardized treatment methods that can limit long-term damage and speed up recovery.

“For stroke care(NOT RECOVERY!), every minute matters,” Loggini said. “From the moment a patient arrives in the Emergency Department, there is a coordinated response involving emergency physicians, nurses, imaging teams and the neuroscience team. That shared focus allows us to move quickly, reduce delays and give patients the best possible chance for recovery.”

Hospital officials said that coordination often begins even before the patient reaches the emergency room.

“This recognition is a direct reflection of teamwork — not just in the Emergency Department, but starting with emergency medical services in the field,” said Jonatan Hornik, MD, Neurointensivist with Southern Illinois Healthcare. “Early identification, rapid communication and seamless handoff between EMS and our ED teams set everything in motion. Stroke care(NOT RECOVERY!) succeeds when every link in the chain works together with urgency and precision.”

Hornik said the Target: Stroke Honor Roll distinction recognizes how collaboration helps teams deliver treatment quickly while maintaining high standards of care(NOT RECOVERY!).

“When EMS, emergency clinicians, radiology and neuroscience teams are aligned, we can move efficiently while maintaining high standards of care(NOT RECOVERY!),” he said. “That collective effort is what allows patients to receive timely treatment and what ultimately drives better outcomes.”

According to Steven Messe, MD, volunteer chair of the American Heart Association Stroke System of Care Advisory Group, hospitals that participate in the Get With The Guidelines program often see measurable improvements in patient outcomes.

“We are incredibly pleased to recognize SIH Herrin Hospital and SIH Memorial Hospital of Carbondale for their commitment to caring for patients with stroke,” Messe said. “Participation in Get With The Guidelines is associated with improved patient outcomes, fewer readmissions and lower mortality rates — a win for health care(NOT RECOVERY!) systems, families and communities.”

Let's check how easy it is to determine stroke hospital competency

How many ways is your hospital incompetent and for how long? Of these 13 listed has your fuckingly incompetent hospital fixed any of them?

Let's count, it won't be pretty and yet your hospital still gets paid for COMPLETE INCOMPETENCY!

Your hospital should have protocols, NOT GUIDELINES for each item. Your board of directors is responsible for setting the standards of running a hospital; DID THEY COMPLETELY FUCKING FAIL AT THEIR JOB?

And stating they follow 'Get with the Guidelines' IS NOT AN ACCEPTABLE ANSWER!

It is simple, solve these problems. Yep they will be hard but leaders tackle the big problems. Are you a leader or a mouse?

1. 30% get spasticity NOTHING THAT WILL CURE IT.

2. At least half of all stroke survivors experience fatigue Or is it 70%?

Or is it 40%?

NOTHING THAT WILL CURE IT.

3. Over half of stroke patients have attention problems.

NOTHING THAT WILL CURE IT.

4.  The incidence of constipation for stroke was 48%.  June 2017

NO PROTOCOLS THAT WILL CURE IT.

5. No EXACT stroke protocols that address any of your muscle limitations.

6. Post stroke depression(33% chance)

NO PROTOCOLS THAT WILL ADDRESS IT.

7.  Post stroke anxiety(20% chance) NO PROTOCOLS THAT WILL ADDRESS IT.

8. Posttraumatic stress disorder(23% chance)  NO PROTOCOLS THAT WILL ADDRESS IT.

  9.  12% tPA efficacy for full recovery NO ONE IS WORKING ON SOMETHING BETTER.

10.  10% seizures post stroke NO PROTOCOLS THAT WILL ADDRESS IT.

11. 21% of patients had developed cachexia NO PROTOCOLS THAT WILL ADDRESS IT.

12. You lost 5 cognitive years from your stroke  NO PROTOCOLS THAT WILL ADDRESS IT.

13.  33% dementia chance post-stroke from an Australian study?

       Or is it 17-66%?

Or is it 20% chance in this research?

NO PROTOCOLS THAT WILL ADDRESS THIS

Association of Component Strategies of the Target Stroke Phase 3 Nationwide Quality Improvement Program With Accelerated Door-to-Puncture and Door-In-Door-Out Times for Ischemic Stroke Endovascular Thrombectomy in the United States

This proves how useless the ASA is! NO measurements of 100% recovery; completely failed business101!

Oops, I'm not playing by the polite rules of Dale Carnegie,  'How to Win Friends and Influence People'.

Telling stroke medical persons they know nothing about stroke is a no-no even if it is true.

Politeness will never solve anything in stroke. Yes, I'm a bomb thrower and proud of it. Someday a stroke 'leader' will try to ream me out for making them look bad by being truthful, I look forward to that day.

Send me personal hate mail on this: oc1dean@gmail.com. I'll print your complete statement with your name(If you can't stand by your name don't bother replying anonymously) and my response in my blog. Or are you afraid to engage with my stroke-addled mind? No excuses are allowed! You're medically trained; it should be simple to precisely state EXACTLY WHY you aren't working on 100% recovery protocols with NO EXCUSES!

Association of Component Strategies of the Target Stroke Phase 3 Nationwide Quality Improvement Program With Accelerated Door-to-Puncture and Door-In-Door-Out Times for Ischemic Stroke Endovascular Thrombectomy in the United States

Circulation: Cardiovascular Quality and Outcomes

Abstract

BACKGROUND:The Target Stroke Phase III program is a national quality improvement initiative led by the American Heart Association, which sought to improve the quality of care(NOT RECOVERY!) for patients with acute stroke undergoing acute reperfusion therapy including endovascular thrombectomy (EVT).

METHODS:

A retrospective, observational cohort study was performed using data from the American Heart Association Get With The Guidelines-Stroke Program between January 1, 2017, and March 31, 2022. Three categories of patients were analyzed: (1) patients who arrived directly at the thrombectomy hospital and had EVT, (2) patients who were transferred in from a nonthrombectomy hospital and had EVT, and (3) patients at a nonthrombectomy hospital who were potentially eligible for EVT, received intravenous thrombolysis, and were transferred out. The primary end point of this study for thrombectomy hospitals was door-to-puncture time.

RESULTS:

In direct-arriving EVT patients, 2 Target Stroke Phase III strategies were independently associated with shorter door-to-puncture time: (1) alerting the neurointerventional team based on emergency medical services prenotification (−21.9 [95% CI, −42.5 to −1.3] minutes) and (2) performance of a brain computed tomography and computed tomography angiography in all patients presenting ≤24 hours from time last known well (−6.6 [95% CI, −11.8 to −1.5] minutes). In transfer-in EVT patients, 2 Target Stroke Phase III strategies were independently associated with a shorter door-to-puncture time: (1) increased use of stroke screening tools (–3.5 [95% CI, −6.4 to −0.6] minutes per 25% increase in use of the screening tool) and (2) increased use of a camera during telestroke consultations (−5.8 [95% CI, −10.7 to −0.9] minutes per 25% increase in camera use).

CONCLUSIONS:

Several Target Stroke Phase III strategies are associated with more timely care(NOT RECOVERY!), which are distinctly different for thrombectomy and nonthrombectomy hospitals and for patients arriving by emergency medical services compared with interfacility transfer.

RECOVERY IS THE ONLY GOAL IN STROKE!

Advanced Certifications, Personalized Care: Inside EAMC’s Stroke Rehab Program

When a stroke turns life upside down, recovery depends on more than determination—it takes a team of specialists with the highest levels of training.

At East Alabama Medical Center (EAMC), the stroke rehabilitation team consists of therapists whose advanced stroke certifications set them apart, combining nationally recognized expertise with compassionate, hands-on care(NOT RECOVERY!) to help patients regain independence and confidence every step of the way.

Stewart Jackson, director of Rehab Services at EAMC, believes that the department’s emphasis on credentialed care(NOT RECOVERY!) is paramount in ensuring stroke patients receive the highest possible care(NOT RECOVERY!).

“We have a department goal to add three new stroke certifications annually, which does not happen without alignment from everyone in this building,” Jackson said. “We want to set the bar for stroke care(NOT RECOVERY!) in the state of Alabama.”

Rehabilitation is a critical component of stroke recovery, providing patients with the tools and support they need to regain strength, mobility, and independence. Through a combination of physical therapy, occupational therapy and speech therapy, the rehab team helps individuals relearn essential skills, improve coordination, and adapt to any lasting impairments.

Within the Rehab Services Department at EAMC, nine individuals hold stroke-specific certifications including four Neurodevelopmental Treatment certifications and five Stroke and Rehabilitation Specialist certifications. These credentials reflect advanced training beyond standard licensure, demonstrating that the team has mastered specialized techniques for restoring movement, function and independence in stroke patients.

“If you have a stroke and come to East Alabama Health, you will be evaluated and treated by rehabilitation professionals who’ve completed advanced hands-on training; that’s a guarantee,” Jackson stated.

EAMC has earned the American Heart Association’s “Get With The Guidelines – Stroke Gold Plus” designation, a mark of excellence in research-based, high-quality stroke care(NOT RECOVERY!). That standard of excellence carries through to the hospital’s rehabilitation team, where certified specialists work alongside neurologists and nurses to provide a seamless, comprehensive recovery experience for every stroke patient.

“We are led by such a great stroke team that values our input and actively collaborates with us in the decision-making process regarding what is the best pathway toward improvement for our patients,” Jackson said. “The collaboration between our stroke providers—Dr. Nojan Valadi and nurse practitioner Jayme Gardner—the nursing staff, and our therapists is where our strength comes from. Everyone plays such a crucial role.”

From therapists to nurses, every member of EAMC’s stroke team brings specialized knowledge and coordinated care(NOT RECOVERY!), ensuring patients benefit from a seamless, expert-driven treatment and recovery experience.
