A higher intake of high-fat cheese and high-fat cream, but not low-fat dairy products, was associated with a lower risk for dementia, independent of lifestyle factors and overall diet quality, results of a large Swedish population-based study showed.
However, the investigators emphasized that the observational study design does not prove a causal link, and outside experts urged caution in interpreting the findings.
“For decades, the debate over high-fat vs low-fat diets has shaped health advice, sometimes even categorizing cheese as an unhealthy food to limit. Our study found that some high-fat dairy products may actually lower the risk for dementia, challenging some long-held assumptions about fat and brain health,” study investigator Emily Sonestedt, PhD, of Lund University in Lund, Sweden, said in a news release.
The study was published online on December 17 in Neurology.

Higher intake of high-fat cheese and cream in midlife was tied to a lower risk of subsequent dementia, a 25-year study in Sweden showed.
Adults who ate 50 g or more of high-fat cheese daily had a 13% lower risk of all-cause dementia (HR 0.87, 95% CI 0.78-0.97) and a 29% lower risk of vascular dementia (HR 0.71, 95% CI 0.52-0.96) compared with those who ate less than 15 g a day, said Yufeng Du, PhD, of Lanzhou University in China, and co-authors.
Those who consumed 20 g or more of high-fat cream daily had a 16% lower dementia risk compared with those who consumed no high-fat cream (HR 0.84, 95% CI 0.72-0.98), the researchers reported in Neurology.
Low-fat cheese, low-fat cream, and other dairy products, including milk (regardless of fat content), fermented milk, and butter, had no significant association with dementia risk.
High-fat cheeses have more than 20% fat and include cheddar, Brie, and Gouda; two slices of cheddar were roughly equal to 50 g. High-fat creams typically contain 30-40% fat and include whipping cream, double cream, and clotted cream.
This study was observational and cannot prove causality, noted co-author Emily Sonestedt, PhD, of Lund University in Sweden. "The most likely interpretation is that high-fat cheese was part of a broader dietary pattern that supported vascular health in this population," she told MedPage Today.
The study results "align with previous research showing that certain fermented dairy products are not harmful -- and may even be beneficial -- for cardiovascular health, which is closely linked to brain health," Sonestedt said.
Nonetheless, the outcomes challenge "some long-held assumptions about fat and brain health," she noted.
The data come on the heels of other research that suggested that the risks associated with high-fat foods may be overstated.
"For decades, public health advice has championed low-fat dairy, a recommendation born from concerns that the saturated fat in whole-fat products would elevate the risk of cardiovascular disease," wrote Tian-Shin Yeh, MD, MMSc, PhD, of Taipei Medical University in Taiwan, in an accompanying editorial.
"More recently, this view has been challenged by publications suggesting that dairy fat may have a neutral association with cardiovascular outcomes, reigniting the controversy," Yeh added. "When the focus shifts from the heart to the brain, the picture becomes even murkier."
The Swedish findings need to be replicated in other populations with varying dietary patterns, she noted.
"To move beyond association and toward causation, randomized trials should test the effects of specific dairy products, such as high-fat versus low-fat and fermented versus non-fermented options, as well as the impact of replacing dairy with high-quality plant-based alternatives on cognitive outcomes," Yeh wrote.
"While the development of dementia requires extended follow-up to capture, trials using cognitive decline or other intermediate outcomes may offer more feasible paths forward," she suggested.
Du and colleagues studied 27,670 participants in the prospective Malmö Diet and Cancer cohort who had baseline dietary assessments that involved a 7-day food diary, a food frequency questionnaire, and a dietary interview.
Participants had a mean baseline age of 58 years, and 61% were female. The researchers identified dementia diagnoses using Swedish National Patient Register data through 2020, validating cases before 2014.
The median follow-up was 24.9 years. During that period, 3,208 incident dementia cases were recorded, including diagnoses of vascular dementia and Alzheimer's disease. High-fat cheese intake was associated with a lower risk of Alzheimer's among APOE4 noncarriers, but not among carriers.
The study had several limitations, the researchers acknowledged. Unmeasured confounding may have influenced outcomes. Diet was assessed only at baseline and changes may have occurred over the follow-up period. The findings are based on Swedish people and may not apply to other populations.
"A major strength of our study is the detailed dietary assessment and validated dementia diagnoses, but we need similar studies in other populations with different eating habits," Sonestedt said.
"For clinicians, the key message is that moderate amounts of high-fat cheese do not appear detrimental to brain health and may fit within an overall healthy diet," she noted.

In a randomized, crossover design study, the researchers found that when adults consumed a high sodium diet, they also experienced blood vessel dysfunction. But, when the same adults consumed four servings of cheese a day alongside the same high sodium diet, they did not experience this effect.
Billie Alba, who led the study while finishing her PhD at Penn State, said the findings may help people balance food that tastes good with minimizing the risks that come with eating too much salt.
"While there's a big push to reduce dietary sodium, for a lot of people it's difficult," Alba said. "Possibly being able to incorporate more dairy products, like cheese, could be an alternative strategy to reduce cardiovascular risk and improve vessel health without necessarily reducing total sodium."
While sodium is a mineral that is vital to the human body in small doses, the researchers said too much dietary sodium is associated with cardiovascular risk factors like high blood pressure. The American Heart Association recommends no more than 2,300 milligrams (mg) of sodium a day, with the ideal amount being closer to 1,500 mg for most adults.
According to Lacy Alexander, professor of kinesiology at Penn State and another researcher on the study, previous research has shown a connection between dairy products—even cheeses high in sodium—and improved heart health measures.
"Studies have shown that people who consume the recommended number of dairy servings each day typically have lower blood pressure and better cardiovascular health in general," Alexander said. "We wanted to look at those connections more closely as well as explore some of the precise mechanisms by which cheese, a dairy product, may affect heart health."
The researchers recruited 11 adults without salt-sensitive blood pressure for the study. They each followed four separate diets for eight days at a time: a low-sodium, no-dairy diet; a low-sodium, high-cheese diet; a high-sodium, no-dairy diet; and a high-sodium, high-cheese diet.
The low sodium diets had participants consume 1,500 mg of salt a day, while the high sodium diets included 5,500 mg of salt per day. The cheese diets included 170 grams, or about four servings, of several different types of cheese a day.
At the end of each week-long diet, the participants returned to the lab for testing. The researchers inserted tiny fibers under the participants' skin and applied a small amount of the drug acetylcholine, a compound that signals blood vessels to relax. By examining how each participants' blood vessels reacted to the drug, the researchers were able to measure blood vessel function.
The participants also underwent blood pressure monitoring and provided a urine sample to ensure they had been consuming the correct amount of salt throughout the week.
The researchers found that after a week on the high sodium, no cheese diet, the participants' blood vessels did not respond as well to the acetylcholine—which is specific to specialized cells in the blood vessel—and had a more difficult time relaxing. But this was not seen after the high sodium, high cheese diet.
"While the participants were on the high-sodium diet without any cheese, we saw their blood vessel function dip to what you would typically see in someone with pretty advanced cardiovascular risk factors," Alexander said. "But when they consumed the same amount of salt, and ate cheese as a source of that salt, those effects were completely avoided."
Alba said that while the researchers cannot be sure that the effects are caused by any one specific nutrient in cheese, the data suggests that antioxidants in cheese may be a contributing factor.
"Consuming high amounts of sodium causes an increase in molecules that are harmful to blood vessel health and overall heart health," Alba said. "There is scientific evidence that dairy-based nutrients, specifically peptides generated during the digestion of dairy proteins, have beneficial antioxidant properties, meaning that they have the ability to scavenge these oxidant molecules and thereby protect against their damaging physiological effects."
Alba said that in the future, it will be important to study these effects in larger studies, as well as further research possible mechanisms by which dairy foods may preserve vascular health.

How many years is it going to take before our medical staff creates a diet protocol that lowers blood pressure? I'm guessing never since prescribing drugs is way too easy and lucrative for pharma.
Ask your doctor, and not politely, 'How much of this cheese should I be eating on a daily basis to reduce my blood pressure 10 points?'
No answer or deflection means you have a doctor that should never be seen again. Somehow we have to force doctors to do their job, which might in the short term mean lots of fired doctors. Their job is to produce results, not just regurgitate old data.

A few tablespoons of Grana Padano cheese every day had the blood pressure-lowering impact typical of antihypertensive medications in a small blinded, randomized trial. A daily 30-g dose of the Italian hard cheese, which is similar to Parmesan, lowered office blood pressure by a mean 6/5 mm Hg more than did a placebo of shredded bread scented to mimic the cheese and with the same fat, calcium, and sodium profile (from 139/88 mm Hg at baseline to 130/81 versus 136/86 mm Hg, P<0.05).The effect on automated office blood pressure readings was a similar 6/4 mm Hg greater with the cheese (P<0.05), Giuseppe Crippa, MD, of Guglielmo da Saliceto Hospital and Catholic University in Piacenza, Italy, and colleagues reported here at the American Society of Hypertension meeting.
Body mass index, total and HDL cholesterol, triglycerides, and serum sodium and potassium were unaffected.
The trial included 30 patients with mild-to-moderate essential hypertension (mean age 54, 13 women), who were on stable pharmacologic and other treatment regimens for the prior 3 months at least.
Participants were randomized in a double-blind, crossover design to 30-g Grana Padano cheese or placebo for 2 months, with individualized dietary counseling for both groups to keep other dietary habits unchanged for the duration of the study.
The amount of sodium in that serving size of cheese -- 128 to 189 mg -- is on par with a similarly sized 1-oz serving of potato chips, Crippa noted.
Fermentation of cow's milk-proteins by cheese-making bacteria produces bioactive oligopeptides that have been shown to have ACE inhibition activity in vitro.
The mechanism isn't far-fetched, as ACE inhibition stemmed from research on snake venom and its vasoactive properties, commented Joseph L. Izzo MD, of New York's University at Buffalo. "It isn't so unlikely that you'd find other biological processes that might be ACE inhibitory."
However, more work is needed both to replicate the findings in a larger cohort and to confirm what mechanism really is at play, he cautioned in an interview with MedPage Today. "No claims should be made."
Also, generalizability to other cheeses isn't clear, Crippa suggested. Whereas Grana Padano is aged 9 to 12 months, fresher cheeses might not have had the time to develop these compounds and longer aging as typical for Parmesan cheese could break them down, he speculated.
Isolating the peptides of interest into pill form, instead of cheese, showed only a small, albeit, significant impact on office systolic blood pressure in clinical trials, Crippa noted, likening the situation to the cardiovascular benefit of red wine versus its resveratrol compound when isolated.
The study also collected 24-hour ambulatory blood pressure, but those results were not presented.
Crippa disclosed relevant relationships with the Consortia Grana Padano.
