Moving beyond single targets in neurological repair

Neuro-Innovators is testing whether a combination of existing medicines can promote neuroplasticity and improve outcomes for people living with chronic stroke disability.

Stroke is a leading cause of long-term disability worldwide, leaving many survivors with persistent impairments despite advances in acute treatment and rehabilitation. A key determinant of post-stroke recovery is the brain’s ability to reorganize its structure and function following injury, known as neuroplasticity. This process involves a complex network of interacting mechanisms, including changes in synaptic connections, neural circuitry, blood vessel formation, and inflammation. However, harnessing these separate mechanisms is extremely difficult.

Neuro-Innovators is taking a multi-pathway approach to that challenge. Rather than searching for a single new molecule, the company is combining existing FDA-approved drugs in an effort to influence multiple biological pathways involved in brain recovery.

The company’s lead program, NIV-001, combines telmisartan, metformin, and cilostazol — three drugs with established clinical histories but different biological activities — and is now being evaluated in an investigator-initiated clinical study at Mass General Brigham’s Spaulding Rehabilitation Hospital. The trial is testing whether the combination, when paired with intensive robot-assisted rehabilitation, can improve motor recovery in people living with chronic stroke-related disability.

“When you look at neuro drugs, the paucity of successful new single molecules is really mind-numbing. Think about the heart, lungs, liver or kidneys — these organs have a fairly fixed operational set, so a narrow, well-targeted mechanism of action can have quite an impact,” Howison Schroeder, CEO of Neuro-Innovators, told DDN. “The brain is much more complex and highly adaptive. You need to come up with something that is going to manage the system rather than simply treat a symptom.”

Moving beyond single-target approaches

Neuro-Innovators evaluated approximately 2,000 compounds with potential neuroplastic effects, including nutraceuticals, psychedelics, and FDA-approved drugs, eventually identifying around 160 candidates.

From this group, the team looked for combinations that could influence multiple aspects of recovery while maintaining an established safety profile. “We optimized both the variety of mechanisms of plasticity, the variety of mechanisms of action within each of those mechanisms of plasticity, and then safety,” Schroeder said.

NIV-001 was designed to affect several biological processes associated with recovery, including inflammation, neurogenesis, angiogenesis, and bioenergetics. Each of the three drugs has shown effects in preclinical or clinical research that could be relevant to neurological recovery. For example, metformin, best known for treating type 2 diabetes, has been shown to promote neurogenesis, reduce neuroinflammation, and support angiogenesis in preclinical studies.

Telmisartan, a well-established antihypertensive drug, has also attracted interest for its effects beyond blood pressure control. The drug can regulate inflammation, oxidative stress, and cellular metabolism within the brain. Additionally, preclinical studies have shown that it can inhibit astrocyte and microglia activation and promote a shift from pro-inflammatory M1 microglia toward the more reparative M2 state, mitigating neuroinflammation and neuronal damage.

Cilostazol is already commonly used for stroke prevention in several Asia-Pacific countries, as an antiplatelet agent. However, preclinical research has also suggested that cilostazol may influence processes relevant to neural repair, including myelin maintenance, and communication between astrocytes and neurons.

In combination, these drugs could provide a way to influence several processes implicated in recovery rather than addressing any single mechanism in isolation. Now, the company needs to test whether those complementary effects can translate into greater functional recovery when the drugs are combined.

Pairing pharmacology with rehabilitation

A key concept underpinning the design of the clinical study is that patients will receive both NIV-001 and intensive robot-assisted upper-extremity rehabilitation. “The heavy hitter is that neurons that fire together, wire together,” Schroeder said. “It is key that [the patients] be doing something with their brain while it’s in this particularly responsive neurobiological state.”

Paolo Bonato, Director of the Motion Analysis Laboratory at Spaulding Rehabilitation Hospital, said the combination of pharmacotherapy and high-intensity rehabilitation was one of the reasons his team was interested in evaluating the approach.

“In the chronic stage, we do see improvements in motor function in response to high-intensity, high-dose interventions, and robotics is a good way to deliver that type of intervention. But the gains are still modest,” Bonato said. “The hope is that by combining pharmacotherapy with high-intensity interventions, we would significantly increase the motor gains that we’re achieving right now.”

The current study is designed as an exploratory investigation, enrolling up to 50 participants and using the Fugl-Meyer Assessment as its primary endpoint. Schroeder said that the team will also collect biological measurements from patients before, during, and after treatment to explore whether specific biomarkers correlate with response.

“If we can calibrate changes in blood markers to positive or negative outcomes, we have an opportunity to identify phenotypes that are particularly responsive to the therapy,” he said.

Building a broader platform

Although stroke is the company’s first clinical focus, Neuro-Innovators views NIV-001 as an initial test of a broader drug-combination strategy.

Stroke provides a relatively well-defined starting point because it is an injury with measurable functional outcomes. Other neurological conditions, including Alzheimer’s disease and multiple sclerosis, present additional challenges because disease progression and biological markers are more complex.

“We’ll also be looking at this sort of indication-wise,” Schroeder said. “Stroke is an injury. Alzheimer’s or MS are neurodegenerative diseases. Can we come up with other ways to alter their outcomes?”

The company is continuing to explore the approximately 160 compounds identified through its initial literature review, with the aim of finding combinations tailored to different neurological conditions.

For now, however, the focus is on whether influencing multiple biological pathways simultaneously can make the brain more responsive to rehabilitation. The answer could determine whether NIV-001 becomes a one-off approach to stroke recovery or the first example of a broader strategy for using combinatorial approaches to promote neurological repair.

Why Is Mobility and Balance Important As We Age?

As we age, mobility and balance are good indicators of our physical health.

“Balance requires your body, brain, and nerves to all work together. This is one of the reasons why balance has been increasingly linked with longevity,” says Mansour. Not to mention, that better balance helps reduce trips and falls, which can lower your risk of injury.

“Start adding balance exercises into your workout routine by simply balancing on one foot for 10 seconds. If that’s easy, close your eyes and try it again. Other exercises to focus on balance include tree pose in yoga, and weighted one-legged exercises for strength training.”

Using mobility exercises to improve our range of motion is important for avoiding aches and pains, moving safely and efficiently through daily activities, and maintaining independence as age. “Doing simple things like arm circles, hugging one knee into your chest, and sitting up with your legs extended and folding forward to stretch the backs of your legs can go a long way towards improving your overall flexibility and mobility,” says Mansour.

Why is Rest and Recovery Important As We Age?

Don’t forget about recovery! “Think about recovery as a ‘thank you’ for all of the hard work during your workout plan,” says Mansour. “It’s so important, especially as we age, because our bodies do so much for us in our daily lives. Stretching, foam rolling, deep breathing, and even massages help sustain our bodies in addition to our workouts.”

Tips for Starting a Fitness Routine That Supports Longevity

  • Ease into it: “Starting a routine too fast and too hard is one way I see people sidelining themselves due to injury or strain because their bodies just aren’t as agile, flexible, or adaptable as they were when they were younger,” says Mansour. “Think about it: over time, all of your repetitive positions like sitting at a desk, lying on your bed, relaxing on a couch, sitting at a dinner table or in the car, etc. take a toll on your body. When you add in day in and day out usage and age, plus repetitive sedentary positions, it’s hard to go from 0 to 100 with a workout plan.”
  • Don’t forget core strength: “Core strength helps to keep the middle of the body strong and tight to reduce the likelihood of lower-back injuries,” says Mansour. “Core strength also helps increase overall strength and balance!”
  • Incorporate power: “Power is how quickly you can apply strength. Training power focuses on exercises like standing up quickly from sitting on a bench,” says Mansour. “Focus on things like quickly walking while carrying a heavy weight or quick jumps. But remember to honor your body’s current state and if lifting heavy objects or doing high-impact exercises are not appropriate for your body, focus on other things like balance and mobility.”
  • Remember that 5 minutes is better than nothing: “I have spent almost two decades working with people primarily age 40 and up. The old school mentality of long workouts, dripping in sweat, and pushing yourself to your limits is what we grew up thinking was right,” says Mansour. “However, that hasn’t motivated all of us to commit to exercise. Dialing it back and thinking about how you can make fitness a part of your everyday life and something that you look forward to is how I’ve helped thousands of people become consistent with their fitness routine.” That’s why she always encourages people to start with just 5 minutes a day. “I do recommend walking because all you have to do is lace up your shoes and get moving,” she says. “After that, add in stretching. And after that, focus on strength training.”

“As we get older, we naturally lose muscle mass, but eating enough protein throughout the day can help preserve strength, mobility, and independence,” says Rizzo. “Instead of saving all your protein for dinner, aim for 25–30 grams at each meal.”

“The natural pigments that make fruits and vegetables colorful are packed with antioxidants that help protect our cells from damage and support heart health, brain health, and healthy aging,” says Rizzo. She suggests setting the simple goal of including at least two different colors of fruits or vegetables at every meal.

“Some easy ways to do that are to add mixed berries to oatmeal or yogurt, toss spinach and chopped tomato into eggs, snack on carrots, peppers, or snap peas with hummus or stir frozen vegetables into soups, pasta, or grain bowls,” she says. “This simple Sweet Potato and Black Bean Quesadilla is one of my favorite colorful meals from the Start TODAY app.”

Feed Your Gut

“Your gut microbiome plays a surprisingly important role in healthy aging, influencing digestion, immunity, and even brain health,” Rizzo says. “One of the best ways to support it is by eating enough fiber. Aim for 25–35 grams of fiber per day by including high fiber foods, like oats, vegetables, especially sweet potato and broccoli, fruits, like raspberries or pears, beans or lentils, and almonds or chia seeds.”

A new study says this simple change may reduce dementia risk by 50%

By this point, we have all heard about the health benefits of walking—from helping manage blood sugar to boosting mood. However, a new study shows a simple change in how you walk may reduce your dementia risk by 50%—and it has nothing to do with walking 10,000 steps per day or for 15 minutes after every meal. Spoiler alert: It has to do with your pace.

The study published in Neurology on July 14 analyzed data from seven studies. Five were aging studies from the Health and Retirement Study International Network of Studies (HRS-INS) that involved approximately 4,000 older adults with an average age of about 84. The other two were smaller studies (the LonGenity Study and RUSH Memory Aging Project (RUSH MAP)) of adults ages 80 and older. The researchers used brain scans or examined post-mortem brain tissue.

"We studied adults aged 80 and older to identify a unique group we call ‘super movers’—people whose walking speed is comparable to that of adults in their 50s and 60s," says Joe Verghese, M.D., the head neurologist behind the new research and chair of neurology at Renaissance School of Medicine at Stony Brook University. "We then compared their cognition with that of other adults the same age to understand what makes them different."

What the study found

The results were drawn from follow-up periods between approximately 3.5 to 5.5 years and showed that the "super movers" had about a 50% lower risk of developing cognitive impairment than their peers and 60% less likely to report a diagnosis of Alzheimer’s disease or another dementia. They also had a slower decline in memory and thinking skills over time, which was measured by memory recall, processing speed, and mental flexibility tests.

"Even more surprisingly, despite having better cognitive function, they had similar levels of dementia-related brain pathology which suggests they may have resilience mechanisms that protect the brain from developing cognitive symptoms," says Dr. Verghese.

For example, “super mover” brains had bigger hippocampi (the part of the brain responsible for memory functioning), according to Erica Weiss, Ph.D., a clinical neuropsychologist on the study and associate professor of neurology at Albert Einstein College of Medicine.

The takeaway

This new research not only supports the idea that you should walk more, but that you should pick up the pace if you can. "Gait speed at 80 years of age and older is one of the best single predictors of overall physiological health because it integrates cardiovascular, musculoskeletal, neurological, and metabolic function simultaneously," says Nir Barzilai, M.D., one of the study’s researchers and a professor from the department of genetics at the Albert Einstein College of Medicine. "What this study captures isn't just ‘movement is good for the brain’; it's that whole-body biological aging pace shows up in how you walk, and that same pace protects every system."

Staying physically active and maintaining mobility should be viewed as an important part of brain health, adds Dr. Verghese. "Regular exercise, managing chronic medical conditions, and paying attention to changes in walking or balance may help preserve both physical independence and cognitive function as we age," he says.

Still, DNA doesn't have to be your destiny when it comes to aging and illness.

Take Dr. Florence Comite. As an identical twin, she's seen firsthand how our unique life experiences can shape our health and, more importantly, what we can do about it.

"If we're identical twins, why do I love sushi and sashimi, and she doesn't really eat fish? I murder plants, and she's an amazing gardener," Comite quipped to Business Insider.

"By understanding all the factors that make up who we are and thinking of it as a proactive approach, you identify risk before symptoms, and you can stop it in its tracks and even reverse it," she said of aging.

Comite is a top longevity doctor, the founder of the Comite Center for Precision Medicine, and author of the new book "Invincible: Defy Your Genetic Destiny to Live Better, Longer."

She's also an expert in epigenetics, the study of chemical changes that influence gene activity, including those that may affect the aging process.

"Being able to turn switches on and off by the choices we make is the way we can give the control back to ourselves, she said.

Comite said that today's decisions can make a huge difference in our ability to thrive for decades to come.

"We begin to show changes of aging below the surface at the cellular level in the 30s. And we can do something about it to really protect our health for life because the earlier we do it, the better off we're going to be," she said.

Comite shared some of the best health changes she's made to her own routine that you can add to your own "health portfolio" for a longer, healthier life.

Ditch the bedtime snacks

Comite said she saw major benefits after triumphing over her own personal health nemesis: the late-night snack.

"I'm a grazer. At night, I would just sit and snack on fruit, chocolate, ice cream, whatever. When I gave that up, and I made myself aware of the fact that I just have to stop eating an hour or two before bed, it was a world of difference," she said.sleep, the time when your body can repair cellular damage to support your metabolism, immune system, and other essential functions. That's why quality sleep is essential for long-term health, helping stave off age-related decline.

"In your 20s, you can sort of burn the candle at both ends, and your body can compensate. That's no longer possible in your 30s," Comite said.

Cut your alcohol intake in half

If you're winding down for the night with wine, whiskey, or beer, you may be doing more harm than good.

Alcohol has increasingly been tied to serious health risks like cancer, heart disease, and dementia. Recent evidence suggests there may be no safe amount of alcohol.

To minimize the risk of drinking, stick to no more than one serving per day for women and two for men, according to the CDC.

Comite said a simple way to cut back is to swap in a glass of plain water between boozy beverages, halving the amount of alcohol you consume.

"If you like alcohol, cut it back. If you drink two glasses, have one glass and a glass of water because alcohol will cause your blood sugar to go up and then go down while you sleep," Comite said.

For more precise insights on your blood sugar, Comite recommends wearing a continuous glucose monitor (CGM), which can give real-time data on your body's unique response to different types of food and beverages.

Combine cardio and strength training

Another way to optimize your longevity routine is to diversify your fitness, Comite said.

"I make time for exercise. Three days a week, I'm in the gym for an hour, and I mainly do weights. There's a little bit of high intensity and aerobic," she said.

Between sets of weighted exercises, Comite said she'll spend some time on the rower for longevity-boosting cardio.

Comite said she also prioritizes high-intensity interval training (HIIT), such as sprint sessions. The right HIIT workout can maximize your time by tapping into key fitness metrics like VO2 max to make the most of every minute you spend exercising.
