The stroke medical world is completely failing at that goal, they don't even have it as a goal. Shortly after getting out of the hospital and getting NO information on the process or protocols of stroke rehabilitation and recovery I started searching on the internet and found that no other survivor received useful information. This is an attempt to cover all stroke rehabilitation information that should be readily available to survivors so they can talk with informed knowledge to their medical staff. It lays out what needs to be done to get stroke survivors closer to 100% recovery. It's quite disgusting that this information is not available from every stroke association and doctors group.

Abstract
Garlic (Allium sativum) has been widely used for culinary and medicinal purposes. Aged garlic extract (AGE) and sulfur‑containing compounds, including S‑allylcysteine (SAC) are well documented botanical active components of garlic. AGE is prepared by the prolonged extraction of fresh garlic with aqueous ethanol and is considered a nutritional supplement with potential to promote human health. SAC is a water‑soluble organosulfur compound and the most abundant component of AGE. Studies have demonstrated that both AGE and SAC can exert neuroprotective effects against neuroinflammation and neurodegeneration. Another bioactive component in AGE is N‑α‑(1‑deoxy‑D‑fructos‑1‑yl)‑L‑arginine (FruArg) although less is known about the metabolic activity of this compound. The main aim of this review was to provide an undated overview of the neuroprotective perspectives of these active garlic components (AGE, SAC and FruArg). Of interest, our studies and those of others indicate that both AGE and FruArg are involved in the regulation of gene transcription and protein expression. AGE has been shown to reverse 67% of the transcriptome alteration induced by endotoxins‑lipopolysaccharide (LPS), and FruArg has been shown to account for the protective effects by reversing 55% of genes altered in a cell‑based neuroinflammation paradigm stimulated by LPS in murine BV‑2 microglial cells. AGE and FruArg can alleviate neuroinflammatory responses through a variety of signaling pathways, such as Toll‑like receptor and interleukin (IL)‑6 signaling, as well as by upregulating the nuclear factor erythroid 2‑related factor 2 (Nrf2)‑mediated oxidative stress pathways known to promote microglial resiliency against neuroinflammation and neurodegeneration. The capability of FruArg to pass through the blood‑brain barrier further supports its potential as a therapeutic compound. In summary, these experimental results provide new insight into the understanding of the neuroprotective effects of garlic components in promoting brain resiliency for health benefits.

Because aged garlic extract (AGE) has been suggested to have a positive impact on vascular elasticity and endothelial function, researchers evaluated the impact of long-term treatment with AGE on cutaneous tissue perfusion. In total, 122 patients with Framingham Risk Scores ≥ 10 were randomized in a double-blinded manner to placebo or 2,400-mg AGE daily for 1 year and monitored. Using laser Doppler velocimetry, cutaneous microcirculation was measured at 0 and 12 months. According to findings, 12 months of AGE supplement may preserve and even improve microcirculation, as measured by laser Doppler velocimetry. The researchers also noted that, hypothetically, preserving or even increasing microvascular blood flow and cutaneous tissue perfusion may facilitate wound healing.

In this year-long, European, multicenter, randomized controlled trial, researchers evaluated the impact of a Mediterranean-style diet (specifically designed according to dietary recommendations for individuals aged ≥ 65 years) on blood pressure (BP) and arterial stiffness among older adults. They measured blood pressure in 1,294 healthy participants (aged 65-79 years) who were recruited from 5 European centers, and assessed arterial stiffness in a subset of 225 participants. The intervention involved the provision of individually tailored standardized dietary advice and commercially available foods to increase adherence to a Mediterranean diet. Habitual diet was continued in the control group, and current national dietary guidance was provided. A total of 1,142 participants completed the trial (88.2%). Following the year-long intervention, the investigators observed a significant reduction in systolic BP, which was evident in men but not women. Overall, this dietary intervention resulted in clinically relevant attenuations in blood pressure and arterial stiffness.

Heart disease in all its forms continues to be the top killer of Americans. Following a heart-healthy diet like this one and getting plenty of exercise can help, but health experts continue to search for even more effective treatment—and they may have found one: A new study suggests that an antioxidant can breathe new life into old and stiff arteries.
In the small study—published in the journal Hypertension—researchers from the University of Colorado Boulder gave ten volunteers between the ages of 60 and 79 a daily supplement called MitoQ (it's a refined version of the natural antioxidant coenzyme Q10); another ten got a placebo. After six weeks, the researchers analyzed the health and size of participants' arteries, then switched the supplement group to the placebo while the others began taking MitoQ.
At the conclusion of their testing, researchers noted when volunteers got the supplement, their arteries regained flexibility. Essentially, MitoQ seemed to reverse artery aging by as much as 15 to 20 years. The study's lead author, post-doctoral student Matthew Rossman said in a press release that "An improvement of that magnitude, if sustained, is associated with about a 13 percent reduction in heart disease." (These are the 5 types of heart disease and their warning signs.)
According to Rossman's co-author Doug Seals, PhD, the study "breathes new life into the discredited theory that supplementing the diet with antioxidants can improve health." He went on to note that "it suggests that targeting... mitochondria may be a better way to reduce oxidative stress and improve cardiovascular health with aging."
Both Rossman and Dr. Seals plan to conduct additional follow-up studies to confirm their findings. Dr. Seals noted that physical activity and proper eating habits continue to be the best ways of avoiding heart disease. "At the public health level, not enough people are willing to do that," he said. "We're looking for complementary, evidence-based options to prevent age-related changes that drive disease. These supplements may be among them."

So Many Dietary Supplements – How Do You Choose?

The only ones I am taking right now are: Tocotrienols and Aged Garlic Extract. I have no clue what is in them because of the fucking stupidity of the US Congress in passing the Dietary Supplement Health and Education Act of 1994 (DSHEA): (DSHEA) defined dietary supplements as a category of food, which put them under different regulations than drugs. They are considered safe until proven otherwise. Caveat Emptor.

This study was conducted to compare the impact of all–extremity high–intensity interval training (HIIT) and moderate–intensity continuous training (MICT) on aortic pulse wave velocity (PWV) and carotid artery compliance in older adults. The following findings were revealed, all–extremity MICT but not HIIT, improved central arterial stiffness in previously sedentary older adults free of major clinical disease. The conclusion derived has significant implications for aerobic exercise prescription in older adults.

Methods

  • Forty-nine sedentary older adults (age: 64±1 years), free of overt major clinical disease, were randomized to HIIT (n=17), MICT (n=18) or non-exercise controls (CONT; n=14).
  • HIIT (4x4 minutes at 90% of peak heart rate interspersed with 3x3 minutes active recovery at 70% of peak heart rate) and isocaloric MICT (70% of peak heart rate) were conducted on an all-extremity non-weight-bearing ergometer, 4 days/week, for 8 weeks under supervision.
  • Aortic (carotid to femoral; cfPWV) and common carotid artery compliance were evaluated at pre- and post-intervention.

Results

  • cfPWV improved by 0.5 m/sec in MICT (P=0.04) but did not significantly alter in HIIT and CONT (P>0.05).
  • Carotid artery compliance improved by 0.03 mm2/mmHg in MICT (P=0.001), while it remained unchanged in HIIT and CONT (P>0.05).
  • Improvements in arterial stiffness in response to MICT were not confounded by changes in aortic or brachial blood pressure, heart rate, body weight, total and abdominal adiposity, blood lipids or aerobic fitness.

Researchers identify two new potential drug targets for treating atherosclerosis


Researchers at Brigham and Women's Hospital have found two new potential drug targets for treating arterial diseases such as atherosclerosis. By using proteomics to screen a vast number of molecules, the researchers identified PARP9 and PARP14 - two members of the PARP family of proteins - as regulators of macrophage activation, which has been linked to arterial disease by systems biology.
Though the mechanisms that activate macrophages, a type of digestive white blood cell that targets foreign cells, remain incompletely understood, previous research shows that macrophages play an important role in the development of atherosclerosis and its thrombotic complications. Masanori Aikawa, MD, PhD, director of the Center for Interdisciplinary Cardiovascular Sciences (CICS) at the Brigham, his research fellow Hiroshi Iwata, MD, PhD, and colleagues studied atherosclerosis on the protein-level to determine which molecules were most involved in the regulation of macrophages.
Once Aikawa and his colleagues narrowed down their search to these two proteins, they silenced each gene in cultured macrophages and found that tamping down PARP14 increased macrophage activation while tamping down PARP9 had the opposite effect.
Aikawa founded CICS and hopes that this hypothesis-generating method can be used to streamline the lengthy process of drug development. Aikawa and CICS are using a more systematic approach which hinges on network analysis; this analysis predicts which pathways are most likely to control their studied effect so that they can prioritize these pathways. Ideally, this process would take a fraction of the time in comparison to searching through each individual pathway unaware of their likelihood of affecting their studied effect.
Aikawa and his colleagues plan to augment these findings to develop targeted therapeutics for atherosclerosis and other diseases.
"Macrophage activation plays a role in not only vascular disorders but also various inflammatory and autoimmune diseases," said Aikawa. "These results could provide important information about the mechanisms of these diseases and help to develop much needed new therapeutics."

PCSK9 Drug Regresses Coronary Plaque 

The PCSK9 inhibitor evolocumab (Repatha) reduces atherosclerosis volume as measured by intravascular ultrasound (IVUS) compared with placebo atop statin therapy, Amgen announced in topline results from the GLAGOV trial.
Along with that positive finding for the primary endpoint at 78 weeks, the trial also met the secondary endpoints of percent and total atheroma volume regression from baseline.
The findings were encouraging, Pablo Corral, MD, a lipid specialist at the Instituto de Clinica Medica in Buenos Aires, and Sekar Kathiresan, MD, of Massachusetts General Hospital in Boston, agreed in a conversation on Twitter.
It might make positive findings more likely from the pending large cardiovascular outcomes trial, due out early in 2017, suggested Ethan Weiss, MD, of the University of California San Francisco, in the same conversation. "Anecdotally, IVUS correctly predicted torcetrapib results," whereas carotid intima-media thickness did not, Weiss added.
However, "I wouldn't declare victory too early," responded Harlan Krumholz, MD, of Yale University in New Haven, Conn., noting that the outcomes trials haven't been stopped prematurely for efficacy.
The phase III GLAGOV trial included 968 patients getting cardiac catheterization, who were on optimized background statin therapy and randomized to double-blind treatment with evolocumab (420 mg monthly) or placebo subcutaneous injections.
Amgen said no new safety concerns emerged in the trial nor was there a difference in treatment-emergent adverse events between groups.
Detailed results are slated for presentation at the American Heart Association meeting in November.term

Abstract

BACKGROUND:

Although several previous studies have demonstrated that aged garlic extract (AGE) inhibits the progression of coronary artery calcification, its effect on noncalcified plaque (NCP) has been unclear.

OBJECTIVE:

This study investigated whether AGE reduces coronary plaque volume measured by cardiac computed tomography angiography (CCTA) in patients with metabolic syndrome (MetS).

METHODS:

Fifty-five patients with MetS (mean ± SD age: 58.7 ± 6.7 y; 71% men) were prospectively assigned to consume 2400 mg AGE/d (27 patients) or placebo (28 patients) orally. Both groups underwent CCTA at baseline and follow-up 354 ± 41 d apart. Coronary plaque volume, including total plaque volume (TPV), dense calcium (DC), NCP, and low-attenuation plaque (LAP), were measured based upon predefined intensity cutoff values. Multivariable linear regression analysis, adjusted for age, gender, number of risk factors, hyperlipidemia medications, history of coronary artery disease, scan interval time, and baseline %TPV, was performed to examine whether AGE affected each plaque change.

RESULTS:

The %LAP change was significantly reduced in the AGE group compared with the placebo group (-1.5% ± 2.3% compared with 0.2% ± 2.0%, P = 0.0049). In contrast, no difference was observed in %TPV change (0.3% ± 3.3% compared with 1.6% ± 3.0%, P = 0.13), %NCP change (0.2% ± 3.3% compared with 1.4% ± 2.9%, P = 0.14), and %DC change (0.2% ± 1.4%, compared with 0.2% ± 1.7%, P = 0.99). Multivariable linear regression analysis found a beneficial effect of AGE on %LAP regression (β: -1.61; 95% CI: -2.79, -0.43; P = 0.008).

CONCLUSIONS:

This study indicates that the %LAP change was significantly greater in the AGE group than in the placebo group. Further studies are needed to evaluate whether AGE has the ability to stabilize vulnerable plaque and decrease adverse cardiovascular events. This trial was registered at clinicaltrials.gov as NCT01534910.

New study shows aged garlic extract can reduce dangerous plaque buildup in arteries

The supplement Aged Garlic Extract can reverse the buildup of deadly plaque in arteries and help prevent the progression of heart disease, according to a new study scheduled for publication in the Journal of Nutrition.
The research, conducted at LA BioMed, found a reduction in the amount of low-attenuation , or "soft plaque," in the arteries of patients with metabolic syndrome who took Aged Garlic Extract. Metabolic syndrome is characterized by obesity, hypertension and other .
"This study is another demonstration of the benefits of this supplement in reducing the accumulation of soft plaque and preventing the formation of new plaque in the arteries, which can cause ," said Matthew J. Budoff, MD, an LA BioMed lead researcher. "We have completed four randomized studies, and they have led us to conclude that Aged Garlic Extract can help slow the progression of atherosclerosis and reverse the early stages of heart disease."
The study involved 55 patients, aged 40 to 75 years, who had been diagnosed with . All the participants underwent screening at the beginning of the study to measure the total coronary plaque volume as well as dense calcium, non-calcified plaque and low-attenuation plaque. The screening was conducted using Cardiac Computed Tomography Angiography (CCTA), a noninvasive imaging technology that accurately measures calcium deposits and in the arteries.
Following evaluation, the participants were given either a placebo or a dose of 2,400 milligrams of Aged Garlic Extract every day. A follow-up screening conducted a year after the initial screening found those who had taken Aged Garlic Extract had slowed total plaque accumulation by 80%, reduced soft plaque and demonstrated regression (less plaque on follow-up) for low-attenuation plaque.

The effects of garlic extract upon endothelial function, vascular inflammation, oxidative stress and insulin resistance in adults with type 2 diabetes at high cardiovascular risk.

If we had any strategy at all for stroke this study would be followed up because of endothelial function. But it won't be because we have NO leadership in stroke and fucking failures of stroke associations. Would this work in stroke patients without diabetes?

Atkin M, et al. – This study tested the hypothesis that Aged Garlic Extract (AGE) may improve endothelial function, oxidative stress, vascular inflammation and insulin resistance in high risk cardiovascular subjects with type 2 diabetes. In this group of type 2 diabetic patients at high cardiovascular risk, 4 weeks treatment with AGE did not significantly improve endothelial function, vascular inflammation, oxidative stress or insulin resistance.

Methods

  • A double blind, placebo controlled cross-over pilot study was performed in 26 subjects with type 2 diabetes who received 1200mg of AGE or placebo daily for 4 weeks with a 4 week washout period.
  • Plasma HsCRP was measured as a marker of inflammation.
  • Plasma TAOS,blood GSH/GSSG and plasma LHP were measured as markers of oxidative stress/anti-oxidant defence.
  • Insulin resistance was measured using the HOMA-IR method.
  • Endothelial function was measured using change in the reflective index (RI) post salbutamol using digital photoplethysmography and urinary albumin/creatinine ratio was measured as a biochemical surrogate.
  • Measurements were taken at baseline and after intervention with AGE or placebo.

Results

  • Of the 26 patients studied (Male 17, Female 9), age was 61 ± 8 yrs (mean ± 1 SD), HbA1c 7.2 ±1.1%, BP 130/75 ±15.9/9.8 mmHg , total cholesterol 4.2 ±0.81mmol/l, triglyceride 2.11 ± 1.51mmol/l, HDL-cholesterol 1.04 ± 0.29mmol/l.
  • The majority of patients were being treated with metformin (59%), aspirin (50%) and statin (96%) therapy. 36% were treated with an ACEI.
  • There were no changes in these therapies throughout the study.
  • Treatment with AGE had no significant effect upon the above metabolic parameters including insulin resistance.
  • Treatment with AGE also had no significant effect on markers of endothelial function (plethysmography), oxidative stress (TAOS, GSH/GSSG, LHP) or inflammation (HsCRP).
